Personalized Health Partners is Hiring a Remote Director of Clinical Operations, Assisted Living, NP or RN

Personalized Health Partners (PHP), a medical practice affiliated with CommuniCare Health Services, is currently recruiting a Director of Clinical Operations for our Assisted Living (ALF/ILF) clinical division. This position will be based out of one of our regional offices and can be partly remote.

Nurse Practitioner preferred, but we will consider strong, seasoned RN candidates.

PURPOSE/BELIEF STATEMENT

The Director of Clinical Operations-ALF/ILF will be responsible for the oversight and planning of the clinical functions and staff of the Assisted Living/Independent Living clinical division of Personalized Health Partners. This will include oversight of nursing staff and providers in partnering AL/IL communities and assisting with PHP clinics. The Director of Clinical Operations will work in conjunction with the Vice President to identify and implement new business and clinical processes and to monitor clinical goals. The Director of Clinical Operations will be a leader in the organization’s strategic clinical, quality, and financial goals.

EDUCATION and QUALIFICATIONS

Required:

  • Registered Nurse Licensure or higher
  • 5 years of clinical experience
  • 5 years in a clinical or operations management role
  • Active, unrestricted state license (RN or FNP)
  • US work authorization
  • 2 years clinical experience as a Registered Nurse or Nurse Practitioner in geriatric, adult or family practice setting
  • Ability to work in a flexible environment and schedule
  • Passionate about providing healthcare to the senior and low-income populations

Preferred Qualifications:

  • Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
  • National certification of AANP or ANCC
  • Active DEA license
  • Experience with coding, ICD-10 and HCC (Hierarchical Condition Categories)
  • Experience or training working in a metrics, results driven healthcare model
  • Be open to professional development through training, obtaining certifications if necessary, and attending team meetings
  • Experience working in value-based care models
  • Training or experience in geriatric, assisted living, and/or skilled nursing setting
  • Experience working in a collaborative healthcare setting to drive positive outcomes and achieve goals
  • Training or experience in outpatient primary care setting

Job Skills:

  • Must have strong oral and written communication skills
  • Must be detail oriented with an ability to work well both independently and in a team setting
  • Exhibit a customer service approach with teams and patients
  • Strong time management skills required. Must be able to prioritize and adhere to competing deadlines while achieving goals
  • Must be willing to travel when needed (50%).

PRIMARY RESPONSIBILITIES AND ESSENTIAL FUNCTIONS:

  • Oversees the daily operations of clinical team
  • Promotes ongoing team education in clinical as well as non-clinical responsibilities
  • Serves as the primary clinical liaison for all clients and key stakeholders
  • Leads, plans, and executes clinical implementation strategies and initiatives in partnership with the operations team for all new clients
  • Serves as the liaison between the AL/IL practice and telehealth software on clinical outcomes, goals, and concerns
  • Provides ongoing performance feedback throughout the year, counsel employees in disciplinary matters, and obtain assistance from Human Resources for disciplinary actions and/or termination.
  • Ensures compliance with regulatory guidelines and system policies/procedures for all activities within the practice
  • Identify opportunities for process and operational improvement
  • Identify/anticipate education opportunities for clinical team
  • Develop relationships with key stakeholders within Medical Practice, AL/IL Facilities, Corporate Leadership and other external partners
  • Follow all state and federal regulations, guidelines, and laws
  • Other duties as assigned.
