PURPOSE:

Under the General supervision of the Housing Manager, the CPP Housing Specialist
is responsible for provide placement and housing consultation, assistance and
resource development for clients transitioning into the community. The CPP Housing
Assistant will also be responsible for attending various meetings, such as DDS
Monthly Meeting, IMD/PDC Meeting, Living Options, CPP Monthly and Referral
Meetings, QA Meetings, Development Meetings, Brilliant Corners, etc. This role will
also advocate for the people we support by connecting housing providers with Service
Coordinators and stay in contact with both to assure that placement is set.

NATURE OF WORK:
Prepare various reports, including DDS SSRS Vacancy Updates, CPP Claims, DDS

Monthly Report
Respond to miscellaneous requests from DDS relating to placement.
Provide information on CPP Placement options when appropriate for individuals (EBSH,

CCH, ARFPSHN)
Provide CPP Vacancy update and need for referrals
Referral updates from providers and vacancy updates will be discussed if new one come

up
Discuss status of pending projects, any renovations needed, or any other HDO related

issues.
Spearhead the referral process by presenting to Living Options and establishing the

appropriate living arrangements, this may include sending referral packets to providers
and working closely with Service Coordinators and Providers.

Will also be responsible for out of area referrals and responding to other Regional
Centers and SSRS request.

Make request for formal denial letters from vendors, when appropriate.
MINIMUM EDUCATION AND EXPERIENCE:


Bachelor's degree in social work, psychology, public health, sociology or similar.
2-3 years of experience as a Service Coordinator, Placement Coordinator, or similar.
Excellent interpersonal and communication skills.
Excellent verbal and written communication skills, including strong organizational

and interpersonal skills.
Intermediate computer applications experience, including Microsoft Word, Excel

and Outlook as well as the ability to learn new computer applications quickly.
Demonstrated competency in providing individual and family support.
Ongoing participation in training is required to stay current on family support, systems

of care and educational rights.

This position will primarily be based in the office; however, remote/hybrid schedules may also
be available.
