Edgewood Center for Children and Families is Hiring a Remote Bilingual Case Manager-TAY/FSP

NOTE: The California Department of Public Health (CDPH) has issued new requirements for health care and congregant care facilities to decrease the risk of COVID-19 outbreaks. Based on the CDPH public health mandate, all candidates for positions at Edgewood Center must provide proof of vaccination. Edgewood Center will consider applicants with ADA and Title VII exemptions on a case-by-case basis.
Mission Statement
Edgewood is the place to begin for any child or youth to get the mental health care, social services, and academic support they need from highly trained and welcoming professionals.
Position- Bilingual Case Manager-TAY-FSP- This position is full-time, non-exempt, Monday through Friday, approximately 9-5, hybrid (remote, community, office).
Program Description
The Transition Age Youth Full-Service Partnership (TAY FSP) Program provides intensive community-based services to 45 young adults (ages 17-25) identified as having the highest mental health-related needs in San Mateo County. The program relies on a diverse staff and innovative program model to effectively meet the needs of this underserved and often marginalized population. Specific supports and services provided by this team include case management and care coordination, mental health treatment (assessment, therapy, medication management, and psychiatry), family support, crisis prevention and intervention, skill building (independent living, vocational, community safety, and emotional/behavioral), socialization and recreational activities, academic support and coordination, employment exploration, and housing support.
Position Description
Working collaboratively with a multi-disciplinary team, the Case Manager, as team lead, provides leadership, oversight, and coordination of their assigned treatment teams. Case managers also address transition age youth needs, support them in accessing benefits and resources, and build upon their natural supports and community involvement in order to decrease isolation and increase independence. Additionally, they provide mentoring and 1:1 coaching of case management and self-advocacy skills to each of their assigned transition age youth.
Bilingual staff members in the TAY FSP Program play an important role in meeting the cultural and linguistic needs of our young adults and their family members. Staff members may use their language skills in conversation, translation, and the creation of documents. Cultural knowledge may be applied to address service delivery, engagement, and outreach, and increase team awareness and understanding
Position Core Functions
  • Responsible for the leadership and facilitation of treatment team meetings, encouraging and modeling collaboration and problem-solving across the multi-disciplinary teams.
  • Coordinate meetings and services needed by assigned clients, tracking the complex case management needs, and completing all corresponding documents.
  • Provide mentoring and coaching of case management and self-advocacy skills to young adults.
  • Provides in-person case management support at appointments, court dates, team or community meetings, and other appointments where support is requested or required.
  • Communicate effectively verbally and in writing to ensure all treatment team members, clients and families, and providers have the information needed to ensure timely access to high quality services.
  • Knowledge of community resources and referral processes, specifically those pertaining to the 16-25-year-olds.
  • Calm and professional response to crisis situations, including the use of mediation and de-escalation techniques.
Essential Functions
  • Completes role-specific documentation and paperwork thoroughly and in a timely manner, meeting expected timeframe and productivity requirements.
  • Regards 17-25-year-old as persons with dignity and competence and engages them as full collaborators in service planning, delivery, and evaluation.
  • Works one on one with assigned young adults at Edgewood locations, in the home, and/or their community settings, depending on the needs and requests and the community support structures.
  • Attends and actively participates in all mandatory meetings, including staff meetings, client team meetings, and treatment planning meeting.
  • Understands and values cultural and racial differences, their alternative perspectives on mental illness, help-seeking and alternative healing practices, as well as lifestyles, goals, family, and community life.
  • Consistently and effectively communicates with all team members (young adults, families, colleagues, natural supports, and providers), on an on-going basis, ensuring others have necessary and up-to-date information about client engagement, symptoms and behaviors, and progress towards goals.
  • Completes individualized plans for each assigned client, ensuring goals, interventions, activities, and progress are all monitored and documented.
  • Cultivates and maintains a structured independent work schedule that allows accessibility to clients, team members and supervisor while in the field.
  • Completes program and agency-specific documentation to meet San Mateo County, Medi-Cal, and The Joint Commission standards.
  • Facilitates a safe and welcoming environment in the workplace, community, and vehicles (agency or personal).
  • Maintains confidential health records of young adults in compliance with the Edgewood, EPSDT, and San Mateo County Behavioral Health standards and regulations.
  • Attends and actively participates in 1:1 supervision; enlists support, requests, and integrates feedback.
  • Maintains positive work relationships in a respectful and collaborative manner.
  • Collaborate and communicate with internal and external stakeholders, in a manner that promotes agency mission, vision, and values; agency, region, and area goals; consumer and family satisfaction; and highest quality care.
  • Other duties as assigned.
Applied Knowledge, Skills, and Abilities
  • High School diploma or equivalent required; Bachelor’s degree preferred.
  • At least 2 years of experience working in residential, educational, or community-based programs.
  • Experience working with at-risk, disenfranchised, and/or minority populations.
  • Demonstrated ability to work with individuals and/or families in a variety of cultural contexts and community settings.
  • Must have a dependable car, valid California driver’s license, and proof of insurance.
Salary Information
This position offers an hourly rate of pay between $27.23 to $27.23 per hour depending on experience.
