What You’ll Do:
The goal of Street Outreach is to make connections to stable housing with tailored services and supports of the client’s choice, such as health and behavioral health care, transportation, access to benefits etc. Street Outreach staff also work with clients to identify safe alternative to unsheltered homelessness. Street Outreach programs are designed to engage unsheltered people at non-traditional settings such as campsites, parks, libraries etc. Position is also responsible for educating potential landlords, the public, and community partners on programs offered by the agency and the impact they have while building support and political will for ending homelessness. Frequent travel throughout the 11-county service area for meetings, presentations, and trainings may be required.
Essential Duties and Responsibilities: Other duties may be assigned.
  • Contact homeless participants within 48 hours of receiving referral and meet with the person experiencing homelessness in non-traditional settings such as campsites, parks, libraries, etc.
  • Practice diversion to access a safe alternative to unsheltered homelessness, and connect the participant to shelter, if applicable.
  • Completes the following: letter of homeless after verifying homelessness, homeless choice voucher application, waiting list enrollment, release of information and other applicable paperwork.
  • Daily and continuous collaboration between other Homeless and Prevention staff, to include but not limited to Housing Intake and Housing Resource Specialists.
  • Ongoing case management during period of homelessness to determine the best pathway to housing based on individual needs. Street Outreach staff remain with the client until client is enrolled in a housing program.
  • Provide housing focused services: Document readiness, Completing rental applications, Coaching on finding housing, Help with HCV paperwork when pulled from waiting list
  • Provide connection and referral to other crucial needs: Health care, Behavioral health care, Benefits & Transportation.
  • Connect participants with additional resources within the agency and community to help alleviate barriers to self-sufficiency.
  • Enter data in the Homeless Management Information System (HMIS) and other relevant data systems.
  • Attend Inter agency Service Team, By name list, Local Planning body meetings and other relevant community activities.

What We’ll Love About You:
An Associate’s degree in Conflict Resolution, Communications, Psychology or Human Services with two years experience similar position with proven organizational capabilities, good communications skills (verbal and written) and advanced knowledge of computer word processing, spreadsheets, data base and participant tracking programs required. Prefer experience in guiding others in their assigned work, college coursework in Business Services or related field required. Two years working with vulnerable population is required.Employees in this position must possess reliable transportation and a valid driver license and have reliable transportation and vehicle insurance in compliance with the State of Michigan.
