Job Summary
The Aging & Disability Services Care Coordinator II’s primary responsibility is to support older adults, individuals with disabilities, and their caregivers to achieve their goals for community and independent living. Care Coordinators will conduct initial assessments with potential clients to confirm program eligibility and identify the client’s goals. Care Coordinators will then work closely with clients and families to navigate the complex social and health services delivery systems, and collaborate with the client’s medical, mental health, social services and other providers to assist the client to maintain the highest level of stability and independence in the community. Clients can qualify for comprehensive case management services through a number of different programs, and therefore Care Coordinators will be trained to work within the policies of these different eligibility groups.
This position will serve clients who are eligible for the Family Caregiver Support Program, Medicaid Alternative Care, Tailored Support for Older Adults, and other care coordination funding through the King County Area Agency on Aging. ADS Care Coordinator I’s will frequently collaborate with other ADS and NH staff, taking direct referrals and serving as a program expert.
