View our video to learn more about TheKey. This RN Care Manager is responsible for providing comprehensive support to clients and their families dealing with the complexities of care and aging. The RN Care Manager has the opportunity to make a positive impact for clients with complex health issues. With compassionate attention and excellent communication, the RN Care Manager will collaborate with clients and their families, providers, and community resources to address the full continuum of our clients’ health care needs using a holistic, person-centered approach. Care management services will assist clients in achieving optimal quality outcomes through advocacy, harmonizing doctors, medications and minimizing the fragmentation of healthcare delivery systems. Required Qualifications Minimum of 5 years of case/care management experience Possession of a valid Florida Registered Nurse license Complex care experience Efficient and Effective computer skills including navigating multiple systems and keyboarding Preferred Qualifications Bachelor’s degree preferred Certified Case Manager (CCM or CMC) Excellent analytical and problem-solving skills Effective communications, organizational, and interpersonal skills Proficiency using Gmail application Strong writing skills, capability to succinctly document all activities with complete nursing notes in secure online charting system. Job Responsibilities/Abilities: Works independently, self-directed, and flexible Serves as the contact, advocate and resource person for clients, families, providers and community partners Evaluates, identifies, and effectively coordinates clinical resources and community resources to meet the needs of clients/families Collaborates with the Director of Care Management and other key stakeholders in the clients’ healthcare in overcoming barriers in meeting goals and objectives Demonstrates a high level of clinical care competency specific to assessment skills, problem solving and critical thinking, solutions-based approach, crisis issues and discharge planning Utilizes strong organizational skills and the abilities to prioritize based on importance and urgency to address client needs and follow through in a timely manner Develops supportive professional relationships with clients, families, providers, and natural supports Demonstrates a holistic and person-centered approach when delivering services to clients Facilitates routine care conferences as appropriate Understands and adapts appropriately to issues related to communication, cognitive or other client barriers Performs initial and periodic client assessments to understand and identify clients’ physical, behavioral health and social determinants strengths, barriers, and clinical needs to evaluate medical history and current status Advocates for clients at home, in the hospital or other facility, and with medical appointments to ensure quality and continuity of care Performs management of medications and complex care through collaboration with appropriate resources and entities Possesses knowledge of local community resources Ability to be on call rotation and cell phone responsive to client calls Creates and implements a care management care plan that addresses client gaps and barriers identified in the assessment that is mutually agreed upon by the client/family Familiar with clinical needs and disease processes for chronic physical and behavioral illnesses (depression, challenging behaviors, Alzheimer's disease, and other disease-related dementias) Develops quantifiable client goals and desired outcomes and promotes the clients’ ability to self-manage to the greatest extent possible Evaluates the effectiveness of the care plan in meeting established care goals; revises the plan as needed to reflect changing needs, issues, and goals.
