Hyatt Family Facilities is Hiring a Remote Restorative Nurse

Willow Springs Care in Yakima, WA is looking for a Restorative Nurse (RN/LPN) to fill a full-time role at our Skilled Nursing Facility. Must have experience with MDS Coordination and Risk Management. Ideal candidate is an RN or LPN with experience in long term care. Work times/days are flexible. Remote is not an option, this is an on-site position.

Job Summary:

The restorative nurse is responsible for providing restorative nursing care to patients residing in the nursing home. The primary role is to develop and implement individualized restorative nursing programs that focus on maintaining or improving the functional abilities of the residents. The restorative nurse collaborates with the interdisciplinary team to promote resident independence and enhance the quality of life.

Duties and Responsibilities:

  • Assess and evaluate the functional abilities of residents through comprehensive physical and cognitive assessments.
  • Develop, implement, and revise individualized restorative nursing care plans based on resident needs and goals.
  • Provide direct care and assistance to residents in performing activities of daily living and functional activities, such as mobility exercises, range of motion exercises, and ambulation training.
  • Monitor and document resident progress, making adjustments to the care plan as needed.
  • Work closely with the physical and occupational therapists to ensure coordination of care and achievement of rehabilitation goals.
  • Educate residents, families, and caregivers about the importance of restorative nursing care and provide instructions on techniques and exercises.
  • Collaborate with the nursing team to ensure appropriate care coordination and communication.
  • Identify and report changes in residents’ conditions to the healthcare team.
  • Maintain accurate and complete records of restorative care provided.
  • Participate in staff meetings, training programs, and quality improvement initiatives.

Qualifications:

  • Valid nursing license
  • Proven experience as a restorative nurse or in a similar role in a healthcare setting.
  • Solid knowledge of restorative nursing principles, methods, and techniques.
  • Excellent assessment skills to identify residents’ functional limitations and formulate care plans.
  • Ability to work independently and as part of a team, while adhering to established protocols and guidelines.
  • Strong interpersonal and communication skills to interact effectively with residents, families, and healthcare professionals.
  • Demonstrated ability to multitask, prioritize responsibilities, and manage time efficiently.
  • Attention to detail and accuracy in documentation.
  • Compassionate and patient-centered approach.
  • CPR certification preferred.
