We at New Vista Nursing Post Acute, are looking for an RN experienced preferred to organize and oversee all nursing to the facility. · The RN Supervisor is responsible for the nursing care of all residents on the units delegated to him/her by the Director of Nurses. He/she functions as the direct supervisor for all charge nurses, C.N.A.’s, R.N.A.’s and other nursing staff as available.
· The RN Supervisor is in charge of the entire facility when the Executive Director, Assistant Administrator or Director of Nurses is off premises.
SUPERVISORY RESPONSIBILITIES:
· The Nursing Supervisor will have Supervisory Responsibility over the R.N.’s’ L.V.N.’s; C.N.A.’s; & R.N.A.’s assigned to his/her unit.
· The Nursing Supervisor will have the responsibility to ensure that his/her staff are providing quality care to the residents assigned to his/her unit.
· The Charge Nurse is assigned responsibility for assessing, planning, implementing and evaluating the nursing care delivered to an assigned group of residents during his/her shift.
· Included within this assignment is the responsibility for supervision and assuring effective communication between him/herself, the resident, other staff, physicians.
SUPERVISORY RESPONSIBILITY:
· The Charge nurse is responsible for supervising all C.N.A. and R.N.A. personnel on his/her nursing station.
· The Charge nurse is responsible for ensuring that quality care is provided to all residents on his/her nursing station.
· The Charge Nurse is responsible for the comfort and safety of all the residents assigned to his/her care.

ESSENTIAL FUNCTIONS
Customer Service
NURSING CARE AND ASSESSMENT:
- Assess residents upon admission in a timely manner.
- Clinically assesses each resident for his or her medical, physiological, psychosocial, safety and educational needs.
- Determines and documents resident level of care upon admission
- Notifies attending physician of critical changes in conditions following admission
- Develops plan of care with nursing intervention designated to meet the identified resident needs
- Demonstrates that the resident plan of care is utilized in providing resident care.
- Monitors all residents for clinical changes of condition and ensures appropriate responses to those changes.
- Supervises subordinate nursing staff and routinely audits nursing documentation for accuracy and completeness.
PHYSICIAN ORDERS:
· Ensures processing of physician written and telephone orders on a timely basis.
· Supervises the implementation of physician’s orders according to resident needs and interdepartmental procedures.
· Demonstrates ability to carry out all treatments and procedures as ordered according to policy and procedures.
MEDICATION:
· Is able to knowledgeably/safely provide all medication as ordered.
· Maintain I.V. certification
· Updates knowledge of medication:
· Review of current journals
· Attendance of meetings and conferences
· Review and use of PDR or information distributed by pharmacy
· Attendance of continuing education classes.
· Reviews documentation of medication administration to include time, date, route and signature.
· Reviews documentation of resident’s response to medication and know allergies.
· Reviews pharmacy consultant’s reports and ensures compliance with all recommendations.
· Supervises teaching of resident relative to medications being administered.
MEDICARE/SUB-ACUTE UNIT:
· Performs all aspects of Medicare skilled nursing service.
· Coordinates care with technical and professional healthcare staff
· Obtains physicians orders for admission to the distinct part and continues to obtain physician orders for clinical care as appropriate.
· Gives skilled nursing care that is guided by the residents’ plan of care.
· Administer medication as ordered by the physician.
· Document care delivered in the residents’ medical record.
· Performs management rounds at least three times per day to check resident status and the care delivery provided.
· Performs clinical care tasks for residents as ordered by the physician. e.g., IV’s, tubes, vital signs & lung sounds etc.
· Passes medication as ordered and performs necessary care related to medication administration, e.g., pulse, B.P., behavior monitoring, fluid retention, bowel and urine elimination.
· Document daily shift charting according to resident condition and assessment status.
· Report changes of condition to the physician and family on a timely basis. Document changes in licensed nurses note and care plan.
· Identifies when an incident occurs & informs appropriate internal staff, family and physicians. Completes appropriate documentation.
Leadership
· Functions as a role model for other RN’s, LVN’s, C.N.A.’s, showing overall responsibility for the residents in the unit.
· Guides subordinate staff in the provision of care that they are mutually responsible for.

· Handles all resident communication and documentation in a confidential manner
· Releases resident medical information in accordance with facility policy and procedures.
· Complies with State and Federal Regulations with regards to record charting and retention.
· Ensures a safe environment for resident’s, staff and visitors
· Ensures that all staff operates medical equipment safely.
· Observe telephone etiquette
· Answers call lights promptly and in a positive manner
· Demonstrates a team approach to care
· Demonstrates awareness of the need for respecting the dignity and privacy of the resident.
· Ability to interact empathetically and supportively to residents and or their families.
· Ability to explain facilities policies and procedures and the interaction with the resident’s physician.
· Serves on various committees of the facility as directed.
· Maintains confidentiality of all pertinent resident care information to assure resident rights are protected.
Communication
· Communicates resident needs to appropriate personnel and refers to other departments when indicated.
· Notifies changes of condition to physician in a timely and appropriate manner.
· Demonstrates a good working relationship with interdepartmental personnel and encourages teamwork to provide quality resident care.
· Communicates all significant changes of condition to physician and direct supervisor.
· Assures completeness of nursing notes promptly and accurately.
· Acts as an advocate in providing safe delivery of care.
