The Port Chester Public Schools seek motivated and dynamic educators who will be responsible for servicing a diverse student population. 

ESSENTIAL DUTIES AND RESPONSIBILITIES 

• Assures that the Full-Service Community School model is carried out within the context of partnership and collaboration through activities that support parental involvement and assures the functioning of all programs (health, mental health, after school, PDS) including site use. 
• Partners with other colleagues in the development, implementation, and evaluation of the full-service community school plan. 
• Provides a full services school environment that promotes home/school connections. 
• Provide family outreach and wrap around services. 
• Provide parent counseling and training as needed. 
• Assesses the needs of school communities to coordinate and procure community services. 
• Provides daily and monthly updates to the Principal, Director and Community School Advisory Board. 
• Communicates and serves as liaison between schools and collaborating community-based partner organizations. 
• Works with various community organizations and county agencies in meeting the various needs of the school community. 
• Assists with grant writing and facilitating on-site facility use by community agencies and other organizations. 
• Create and collaborate on a community school newsletter for families, community members, and collaborating organizations. 
• Represents the school at community meetings and events. 
• Participates actively in school wide planning and governance teams. 
• Develops/updates instruments to gauge partnership effectiveness using identified metrics within the designed instrument. Conducts need assessments/self-studies to develop an annual plan in collaboration with the principal, lead agencies, and advisory committees with the goal of improving programming outcomes. Ensures the annual plan is aligned with school improvement plan/goals and district initiatives. 
• Conducts parent/family workshops that focus on providing families with information on resources to help improve their quality of life or assist in the education of their children. 
• Coordinates and oversees social services for families as needed. 
• Assists in student social development and other student matters as necessary. 
• Participates in other areas of the school and district as necessary. 
• Develop and implement innovative strategies to support building attendance needs, partnering with parents and community-based organizations to increase student attendance in school.
• Investigates serious attendance problems and custodial residency cases as assigned. 
• Initiate workshops, campaigns, programs and/or other support for the betterment of students and their families. 
• Composes and maintains case records on assigned caseloads; Maintains up-to-date and accurate records on community interactions and engagements. 
• Performs other duties as assigned by the building principal or district administration. 

QUALIFICATIONS REQUIRED 

The position requires current NYS certification in the area of School Social Worker. Fluency in Spanish is required.
