As we continue to grow, we are looking for “SNF-ists,” in-house nurse practitioners that will provide care for the geriatric population in a Skilled Nursing and/or Assisted Living setting.
What You Will Do
As the SNF-ist, you will act as a Skilled Nursing Facility’s leading provider of internal medicine services. You will collaborate with the facility’s interdisciplinary team and physicians, serving as a day-to-day clinical leader while helping oversee the care of all the facility’s residents.
You will work with the entire facility’s patient population - short and long-term residents - overseeing their day-to-day care with the assistance of the facility’s nursing staff and guidance from the facility’s medical directors and attending physicians.
As a SNF-ist, you will provide types of care, including but not limited to:
· Be comfortable and proficient with documentation and appropriate coding
· Evaluation of new-admission visits
· Coordination of discharge visits
· Monthly stability visits for long term residents
· Evaluations for patients exhibiting changes in condition
· Advanced Care Planning
Other Duties & Responsibilities
· Provide comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol in the direct management of acute and chronic illness and disease
· Prescribe all necessary medications and treatments
· Perform comprehensive physical examinations of patients
· Develop and implement patient management policies and procedures
· Facilitate referrals to other healthcare professionals and medical facilities
· Communicate with attending physicians and/or specialists regarding patient care
· Be comfortable and proficient with documentation and CPT coding

Our Requirements
Licensure / Certification / Registration:
· Advanced Practice Licensure (APN) with an applicable state license
· Must have an active license and DEA #
· Previous experience in nursing or other medical fields
· Minimum one-year experience in a Skilled Nursing setting (preferred)
· Ability to build rapport with patients and their loved ones
· Ability to thrive in a fast-paced environment
· Excellent written and verbal communication skills
· Strong leadership experience with emphasis on teamwork in a healthcare setting
