**Qualifications:**
- Active Licensed Vocational Nurse (LVN) license in the state of practice.
- Previous experience in hospice or palliative care is preferred.
- Strong communication and interpersonal skills.

- Ability to work effectively in a team environment.
- Basic computer skills.
**Responsibilities:**
- Collaborate with the interdisciplinary team to develop and implement individualized care plans for patients.
- Administer medications and treatments as prescribed by the physician.
- Monitor and assess patient's condition and report changes to the healthcare team.
- Provide emotional support and education to patients and their families.
- Ensure all documentation is completed accurately and in a timely manner.
- Maintain patient confidentiality and privacy at all times.
- Follow all regulatory standards and company policies.
