Part time, Limited Term position with estimated duration from approximately August through November 2024. East Clinic. There are no benefits for this limited term position.
The Registered Dietitian is responsible for the development, promotion, coordination, and implementation of dietary education to GHC-SCW members across the GHC-SCW delivery system. These initiatives are accomplished through individual appointments, group classes, targeted case management and phone call follow-up. Counseling and instruction will focus on diet, exercise and lifestyle change to maintain/enhance health or improve management of acute or chronic health conditions through dietary modifications. The outcome of the initiatives will be positive interactions with groups and individual members for improving health status, decreasing behavioral risks, and improving control of chronic disease. In addition to other assignments, the incumbent may provide direct nutritional education and case management to patients with diabetes and pre-diabetic conditions such as metabolic syndrome. Work is performed under the general direction, guidance, and supervision of the Clinical Health Education and Integrated Care Manager.
