Required:

- 3 years of experience in:
- Value-Based Care.
- Primary Care.
- Willingness to conduct home visits as required.
Preferred:

Position Summary:
The licensed Nurse Practitioner will spearhead the medical aspects of team-based care for individuals with serious mental illness. This role involves documenting and addressing the medical requirements of individuals through comprehensive clinical assessments aimed at identifying and rectifying care gaps. Additionally, the Nurse Practitioner will coordinate care among medical and behavioral health providers, provide clinical support to Guides and Community Resources Guides, and lead the medical component of team-based care.
Responsibilities:
- Conduct comprehensive clinical assessments, including gathering information from external sources, assessing both behavioral and physical health conditions, and making individualized recommendations or referrals.
- Lead the medical component of team-based care, including training Guides and Community Resources Guides, supporting care coordination, identifying actionable care gaps, and communicating with care providers.
- Demonstrate empathy, compassion, and approachability, alongside excellent communication and interpersonal skills.
- Exhibit comfort in working within a multidisciplinary team and embracing varied roles and responsibilities.
- Possess cultural humility, trauma-informed care knowledge, and understanding of social determinants of health.
- Embrace data-driven care practices and maintain familiarity with local providers.
- Maintain professional boundaries and engagement skills in challenging environments and non-traditional work conditions.
- Demonstrate intermediate computer skills, including familiarity with Microsoft Office Suite, and possess a driver's license, private auto insurance, and insurability under company coverage.
Experience:
- 5 years of clinical care experience as a Nurse Practitioner, preferably in a primary care setting.
- Experience working with and managing underserved populations facing challenges such as behavioral health and/or Substance Use Disorder (SUD) conditions.
- Background in developing and delivering educational programs to nurses and affiliated medical staff, as well as care management and coordination.
