Job Description:

Responsibilities:

• Client Assessment and Care Planning:
• Conduct comprehensive assessments of clients' needs and preferences.
• Develop personalized care plans in collaboration with clients, their families, and healthcare professionals.
• Regularly review and update care plans to reflect changes in clients' conditions or preferences.
• Caregiver Recruitment and Training:
• Recruit, interview, and select qualified caregivers based on client needs and compatibility.
• Coordinate caregiver training sessions on relevant topics such as client safety, infection control, and specialized care techniques.
• Provide ongoing support and supervision to caregivers to ensure adherence to care plans and quality standards.
• Scheduling and Coordination:
• Schedule caregiver visits and ensure adequate coverage for all clients' needs.
• Coordinate with clients and caregivers to accommodate schedule changes, emergencies, and special requests.
• Communicate effectively with clients, caregivers, and healthcare professionals to facilitate smooth coordination of care services.
• Client and Family Support:
• Serve as the primary point of contact for clients and their families, addressing inquiries, concerns, and feedback in a timely and empathetic manner.
• Provide guidance and resources to help clients and families navigate the home care process and access additional support services as needed.
• Conduct regular check-ins with clients and families to assess satisfaction and identify areas for improvement.
• Documentation and Reporting:
• Maintain accurate and up-to-date client records, including care plans, progress notes, and incident reports.
• Prepare regular reports on client status, caregiver performance, and operational metrics for management review.
• Ensure compliance with all regulatory requirements and company policies related to documentation and reporting.

Qualifications:

• Bachelor's degree in social work, nursing, healthcare administration, or related field (preferred).
• Proven experience in home care coordination, case management, or a similar role.
• Strong interpersonal and communication skills, with the ability to build rapport and collaborate effectively with diverse stakeholders.
• Excellent organizational skills and attention to detail.
• Knowledge of home care regulations, best practices, and community resources.
• Ability to work independently, prioritize tasks, and solve problems creatively.
• Proficiency in computer applications, including electronic health records and scheduling software.
• At least 1 year of experience

Job Type: Full-time

Pay: $25.00 - $30.00 per hour

Expected hours: 40 per week
