JOB
The purpose of this position is to provide case management services and assessment of need to persons with developmental disabilities, both adults and children, and disability waiver case management. Social Workers establish collaborative relationships with people served, their families and/or legal representatives, service providers, and other agency staff. They participate and assist with the implementation of unit and agency goals as well as policies and procedures to provide optimal services to people based on resources available to the agency.
EXAMPLE OF DUTIES
Provide Rule 185 case management services in accordance with Minnesota Statutes and Rules and Department of Human Services policy as reflected in various bulletins, memorandums, and service manual. This service is to be provided to eligible adults and children who are developmentally disabled or meet criteria for related conditions and are applicants for or recipients of any program or service offered through the developmental disability programs. Rule 185 case management for persons with developmental disability or related conditions assists persons in gaining access to needed social, medical, educational, and other supports and services. The case manager works on behalf of the person to identify their unique needs and to minimize the impact of the disability on the person’s life while assuring continuity of services and supports for the person.Determine immediate risks to health and safety and take appropriate action.Conduct formal assessments and screenings to collect, compile and analyze information to address the wants and needs of the person served.Provide person centered case management to assist families and individuals through appropriate use of county and community resources.Consult on Long Term Care/MnCHOICES assessment with Certified Assessor.Complete necessary forms for case opening.Develop with the person their Community Services and Support Plan, make referrals and facilitate services through providersAssess risk of institutionalization. Delegate/coordinate responsibilities of team members and providers and monitor services to ensure that the person’s needs are being met.Coordinate with the MnCHOICES Certified Assessor for annual reassessment of eligibility.Conduct annual meetings with Disability Waiver recipients served on case management caseload.Conduct semi-annual review for Disability Waiver recipients.Discuss/reauthorize services provided and delivered with service provider(s).Update person’s case file and service agreement.Monitor and evaluate the delivery of services and adjust to meet the person’s continuing/changing needs.Provide service coordination for Community Support Grant recipients.Maintain case file records and documents. Inform people served of program/service options and process to access.Interpret laws/rules/policy.Preparation, documentation and testimony in court actions.Represent unit, department or agency in community assignments and groups. Act as designated legal representative for wards of county/state by determining/authorizing services for medical, education, residential, advance directives and end of life issues as appropriate and assigned.Consult with agency vulnerable adult worker on all reports of suspected abuse and/or neglect; and assist in investigation when determined appropriate. Provide permanency planning services to children who receive services in out-of-home placement.Perform other duties as assigned.Administrative tasksAttend staff and supervisor/supervision meetings as scheduledAttend conferences as scheduled or agreed upon with supervisorAttend trainings as related to job responsibilitiesComplete documentation, agency timesheets, and employee records in a timely mannerCommunicate schedule changes with supervisorUtilize the status (In/Out) Board consistently to keep agency staff aware of availabilityUtilize SSIS as the case management system, including time documentation, case notes, documents and other systems as applicable. May assist with duties and responsibilities as outlined in the County Emergency Operations Plan and Continuity of Operations Plan.
SUPPLEMENTAL INFORMATION
This position is regularly scheduled to work 40 hours per week, Monday-Friday, 8:00-4:30 p.m. The 2024 starting salary for this position is $32.30 hourly, depending on qualifications, with career advancement to $51.23.The selection process will include an online employment application and supplemental questionnaire. Please note: This position is considered a safety-sensitive position due to the essential job duties in accordance with Minn. Stat. §181.950, Subd. 13. Safety-sensitive employees are subject to pre-employment drug testing and random drug and alcohol testing. In addition, all employees are subject to drug and alcohol testing upon reasonable suspicion/cause, return-to-duty, follow-up, and post-accident.
