Hampton Roads Community Health Center is Hiring a Behavioral Health Consultant Near Portsmouth, VA

The Hampton Roads Community Health Center is looking for Behavioral Health Consultants to join the Behavioral Health Team. As a Federally Qualified Health Center (FQHC) with community health centers in Norfolk and Portsmouth, we are committed to health equity and behavioral health. Under general supervision, this exempt position is responsible for identifying, triaging, and managing patients with medical and behavioral health problems within the primary care setting in addition to performing a variety of assessments, treatment planning, counseling, education, and service coordination. Reports to the Manager of Behavioral Health.

Conducts the initial face-to-face assessment, documents the face-to-face clinical supervision and review in the patients record; reviews medical, educational, and psychological information to form behavioral and cognitive interventions; develops an intervention plan and implements this plan with the provider, patient, and family/caregivers; and communicates directly with psychiatrist and PCP regarding patient’s progress towards goals.

Maintains updated and accurate caseload based upon the volume of patients; verifies patient’s statements, completes evaluations and review sheets, sets up records and files; establishes plans for patients in self-sufficiency goals for the Providers on-going consultation on cases in real time; providing supportive services and establishing solutions for participation barriers; and possesses knowledge of basic human behavior with a working knowledge of practices of public service organizations.

Identifies, enrolls, and tracks patients enrolled in the substance use collaborative care program, per department protocols and policies; identifies patients with a diagnosis of major depressive disorder who have not demonstrated improvement in their PHQ-9 scores and works to enroll them in the Collaborative Care program; provides assessments in real time, while the patient is in the office, seeing the primary care provider or psychiatrist as appropriate; and screens for substance use disorders and refers to appropriate agencies as appropriate.

Provides short term intervention and treatment using evidence-based practices; provides verbal and written consultation to referring physicians/providers and other clinical staff on behalf of their patients; promotes early identification and intervention, and improve motivation for disease management (i.e. obesity, diabetes, hypertension); and works with physician and support staff to develop a common treatment plan for the patient.

Develops and recommends either internal or external Behavioral Health options; plans, organizes, implements, and evaluates organizational risk control plans to minimize adverse impacts to the organization’s strategic goals; and develops, supports, and implements improvements to the departmental organizational climate, leadership development, and strategic initiatives.

Establishes and maintains effective working relationships with others in a positive and tactful manner under sometimes stressful situations; and exercises sound judgment, discretion, tact and resourcefulness in solving problems and drawing logical conclusions.

Performs administrative support work such as word processing, creating spreadsheets, data entry and retrieval; orders, monitors and distributes program supplies/resources; and other information to ensure accuracy and conformance to established procedures and policies; and counsels patients regarding patient responsibilities and rights; adheres to patient confidentiality guidelines. 

Performs other duties as assigned.

Master’s degree from an accredited school. 

Must be a licensed or license-eligible in the Commonwealth of Virginia

Licensed Clinical Social Worker (LCSW) and at least three to five years of related experience. 

Possession of a current unrestricted license to practice in the Commonwealth of Virginia. 

Additional Requirements

Requires an acceptable background check to include state and local criminal checks; satisfactory results from a pre-employment medical evaluation and substance abuse testing and is subject to random alcohol and controlled substance testing. As a condition of continued employment, you must complete a state mandated application for credentialing and recertification.

Required Knowledge

  • Behavioral HealthUtilize therapeutic theories to impact behavioral change with patients for physical health as well as mental health. Understanding of how medical diagnoses can impact a person’s mental health. 
  • HIPAA – Knowledge of the federal Health Insurance Portability and Accountability Act (HIPAA) to ensure the privacy and safeguard patient information. 
  • Customer Service — Considerable knowledge of principles and processes for providing customer service. This includes meeting quality standards for service.
  • Technology — Knowledge of general office equipment and personal computers to include word processing, spreadsheet, and related software. Comprehensive knowledge of office systems, practices, procedures, and administration.

  • Computer Skills – Utilizes a personal computer with word processing, spreadsheet, and related software to effectively complete a variety of financial tasks with reasonable speed and accuracy.
  • Time Management - Plans and organizes daily work routine. Estimates expected time of completion of elements of work and establishes a personal schedule accordingly. Implements work activities in accordance with priorities and estimated schedules.
  • Interpersonal Relationships – Develops and maintains cooperative and professional relationships with patients, employees, and all levels of management to include representatives from other departments and organizations.

Required Abilities

  • Communication – Ability to effectively communicate ideas and proposals verbally and in writing, to include the preparation of detailed logs which include numerical information.
  • Judgment/Decision Making — Evaluates the best method of research and then exercises appropriate judgment in establishing priorities and resolving complex matters. Considers the relative costs and benefits of potential actions to choose the most appropriate one.
