Description

Catholic Charities is seeking candidates for the position of a Referral Coordinator to work in our Pregnancy, Parenting and Adoptions program to assist with a new grant we have received from the State of Tennessee to work with pregnant women and families with children under 24 months old. This is a four day a week position with benefits and based in Davidson County. Agency hours are Monday through Friday, between 8:00am to 4:30pm. Office location is 2806 McGavock Pike in the Opryland area of Nashville. 

If you’re looking for an opportunity to join a dynamic team of Clinicians and case managers who are innovative and devoted to the health and healing of clients, then this is the job for you! Primary responsibilities for this Referral Coordinator position is to be the “information hub” for all the programs that exist within Catholic Charities to ensure a smooth transition between program services for our mothers-to-be and their young families to ensure that they are provided with optimal resources to meet their need. This position will be an integral part of the team and the public facing of our program and needs to have great communication skills and the ability to work with a variety of clients and other professionals. This position will also have some minimal administrative responsibilities of assisting with other administrative needs for the program such as scheduling, ordering basic needs supplies and billing for referred services for the program. 

 Job Responsibilities:

  • Coordinate all referrals for program participants and scheduling of appointments as needed.
  • Provide clients with the necessary social service assistance, and referrals to appropriate internal resources and have knowledge of other community resources not offered by Catholic Charities. 
  • Attend community events and provide outreach to the community about the program as needed.
  • Maintains complete and timely case notes according to Agency guidelines, using the database provided by the agency and within Agency time frame.
  • Assist other team members on an as-needed basis.
  • Actively participates in supervision sessions, periodic team meetings, and training.
  • Performs other duties as assigned.

Requirements

Education/Experience:

  • Bilingual English/Spanish Speaking candidate is highly encouraged to apply. 
  • Bachelor’s degree in social work or related field with experience in working with clients from diverse communities.
  • 2 years’ experience working in social services is preferred.
  • Basic case management skills relative to the needs of families experiencing multiple problem areas such as housing, financial stability, and student achievement.
  • Must possess a valid driver’s license and have reliable transportation.

Other Skills or Requirements:

  • Ability to work with individuals experiencing a crisis.
  • Computer proficiency with Microsoft Office applications; ability to learn databases.
  • Knowledge of community or other available resources that serve the needs of families who are experiencing multiple problem areas.
  • Ability to exercise good judgment, courtesy, and tact in dealing with the public and staff.
  • Ability to handle confidential information with extreme professionalism.
  • Ability to plan, organize, and work well under pressure and/or meeting deadlines.
  • Schedule flexibility to accommodate the needs of client.
  • Ability to follow tasks through to completion.
  • Effective interpersonal skills.
  • Strong written and verbal communication skills.
  • Ability to work well as a team and independently with minimal direction.
