Description

NATURE OF WORK: The Assertive Community Treatment Specialist functions as a member of the multidisciplinary team and participates in treatment, rehabilitation, and support services to program clients with severe and persistent mental illnesses. This team member works as an extension to other team members and has the responsibility to carry out rehabilitation and support functions and assists in treatment; provides education; supportive services; training and assistance with ADLs; and provides consultation to families under the clinical supervision of staff with professional degrees.

ESSENTIAL DUTIES:

  • Collaborates with team members to assess, plan, develop, coordinate, and provide treatment, rehabilitation, and support services to program clients.
  • Consults with community agencies and families to maintain coordination in the treatment process.
  • Ensures all clients of the program are seen face-to-face by a team member at least every seven days.
  • Participate in the client-centered comprehensive assessment of individual clients.
  • Participate in daily staff organizational meetings and treatment planning review meetings (IDTs).
  • Participate in providing treatment and dual diagnosis services.
  • Participate in the provision of rehabilitation services.
  • Perform job coaching, problem solving, and support on and off the job site.
  • Provide work-related supportive services, such as assistance with grooming and personal hygiene, securing of appropriate clothing, wake-up calls, and transportation.
  • Provide ongoing assessment, problem solving, side-by-side services, skill training, and environmental adaptations to assist clients with activities of daily living.
  • Assist and support clients in finding and maintaining a safe and affordable place to live and to procure necessities.
  • Uses clear communication skills, including people first language with clients and other professionals.
  • Completes and submits documentation into the Electronic Medical Record (EMR) as required by Valley, Medicaid, Medicare, Private Insurances and any other licensing agencies. Ensures that documentation (such as Direct Services Progress notes, Client Transportation Notes, etc.) are completed accurately and timely and will reflect services the client has received as appropriate.
  • Participates in IDT meetings and staffing meetings.
  • Provide nutrition education and assistance with meal planning grocery shopping, and food preparation.
  • Assist and support clients to perform household activities, including house cleaning and laundry.
  • Teach money-management skills and ensure that clients have adequate financial support.
  • Help clients to access reliable transportation.
  • Assist and support clients to have and effectively use a personal primary care physician, dentist, and other medical specialists as required.
  • Plan, structure, and prompt social and leisure-time activities and provide side-by-side support and coaching to help client socialize.
  • Attends all mandated trainings related to profession and enhances knowledge and/or skills, which include Essential Learning Trainings.
  • Adheres to Valleys Standards of Conduct and Compliance and adheres to professional code of ethics in performing services.
  • Promotes appropriate community interactions by handling public interactions in a professional and competent manner. Returns telephone calls and requests for information in a timely manner, promotes a positive image of the Program and the Agency, and maintains a good rapport with agency contacts.
  • Transport Clients out in the community as needed to complete Activities of Daily Living (Grocery Shopping, Community integration/meetings, etc.) using Company Vehicle or Personal Vehicle if a company one is not available.
  • OTHER DUTIES AS ASSIGNED

Qualifications


MINIMUM QUALIFICATIONS:

  • Ability to perform Essential Duties as outlined below.
  • BA/BS in a Human Services Field.
  • Six months experience working with individuals in human services or related training.
  • Ability to comply with Clients Rights.
  • Ability to comply with all of Valleys Policies and Procedures.
  • Ability to comply with Valleys and Division departmental safety procedures.
  • Ability to read, write, understand and speak the English language.
  • Current CPR/First Aid Certificate or ability to be trained.
  • Valid Drivers License
  • Ability to use E-Mail and Computer to enter documentation into Electronic Medical Record (EMR)

PREFERRED QUALIFICATIONS:

  • Experience working with Assertive Community Treatment Teams.
  • Six months to one year work experience with adults with severe and persistent mental illness.
