OSDTN is Hiring a Veteran Transitional Housing Case-Community Manager Near Nashville, TN

Position: Full Time / Regular / Non-Exempt

Supervisor: Director, THP

SUMMARY: The Transitional Housing Program (THP) Case / Community Manager (CCM). The CCM is responsible for providing a full range of case management services to eligible Veterans, as well as facilitation of the day-to-day program services and operations.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

Case Management:

  • Facilitates and coordinates Veteran’s access to and appropriate use of OSDTN, VA, and/or community resources and services (public/private) to successfully address personal issues/barriers, stabilize personal finances, and secure permanent housing.
  • Assists in the process of securing and maintaining needed eligibility documentation and/or applications for relevant housing/financial activities.
  • Participates with team to identify, develop, and implement best practices for services.
  • Maintains needed documentation via ClientTrack (client management software) of services rendered to include contacts, units, progress notes, assessments, care plans and outcomes.
  • Maintains work consistent with Department/Team Quality Assurance Standards.

Community Management:

  • Facilitates with Deputy Director to manage day-to-day operations.
  • Facilitates evening program schedule.
  • Facilitates completion of the Daily THP log.
  • Facilitates admissions with Director/Deputy Director.
  • Facilitates and coordinates daily agency provided transportation.

General Duties:

  • Assists with case management coverage for Veterans during vacancies and/or PTO.
  • Adheres to Agency and Department policies and procedures.
  • Participates in Agency and Department meetings and events, collection of data/information, and Quality Improvement Monitoring and Review.
  • Performs other duties as designated by supervisor or agency.

QUALIFICATIONS:

Bachelor’s degree in human services and/or commensurate one or more years of experience in direct delivery of social services. Knowledge of veteran issues regarding substance abuse or behavioral health issues and/or experience with transitional housing programs desired. Possesses valid, current Driver’s License

OTHER SKILL, ABILITIES, AND QUALIFICATIONS:

  • Veteran preferred.
  • Excellent organizational, multi-tasking and time management skills.
  • Broad capacity for creative problem solving.
  • Compassionate, energetic and team focused.
  • Comfortable in individual, group and public speaking environments.
