Job Description
Provide supervision, support, and monitoring for safety concerns.
Provide crisis

prevention/intervention

services to the people served.
Monitor the persons supported for symptoms of decompensation, crisis, and medication side effects, and intervene when necessary.
Incorporate the CSSP and CSSP Addendum into daily interaction with the persons served.
Assist the persons supported with problem-solving different aspects of independent living.
Maintain complete and proper documentation for direct service hours, and fully complete documentation prior to leaving a scheduled shift.
Participate in scheduled and spontaneous recreational events (while still following the Enriched Living Transportation Policy).
Maintain confidentiality by complying with HIPAA and MN Data Privacy.
Adhere to your weekly schedule, arrive on time, etc.
Attend mandatory staff meetings, and other meetings requested by PC.
Maintain clear, open, and honest communication with program management.
Other duties as assigned.
