Provides care to Hospice patients and families in their place of residence. Develops initial Plan of Care with patient/ family and team to keep patient safe and comfortable. Supervises Hospice aides. Case Collaborates with interdisciplinary team and hospice physician to manage end of life symptoms and provide holistic support. As a St. Luke’s Hospice nurse, you would be responsible for: Providing comprehensive nursing care to the hospice patient and guidance and supervision to the patient and family in the patient’s residence. Considering psychosocial, environmental, spiritual and financial needs. Completing a holistic assessment of the patient and family. Reviewing the plan of care with the interdisciplinary team and the hospice physician to document and make changes in the plan of care that will assist the patient and family in meeting their end of life goals. Communicating and educating the patient and family on their plan of care. Our St. Luke’s Hospice Program is supported by a 10 bed inpatient unit, a dedicated after hours staff, and a complimentary options program. PHYSICAL AND SENSORY REQUIREMENTS: Physical Demands: Heavy Work Exertional activity. Occasionally lift and/or carry 100 lbs., frequently lift and/or carry 50 lbs., and stand and/or walk at least 6 hours in an 8-hour work day. QUALIFICATIONS (MINIMUM) EDUCATION: A graduate of an approved school of professional nursing who is licensed as a Registered Nurse by the State in which practicing, a Baccalaureate degree preferred.
