Essential Job Functions
  • Assist residents with activities of daily living, including bathing, dressing, eating, toileting, hygiene and mobility. This includes assist lifting and transfer of residents with or without assistance. Practice safe body mechanics and utilize two-person or three-person assist as needed. Utilize safety equipment provided (gait belts, transfer boards, etc.)
  • Capture and record vital signs, including weight, according to policy & procedure and report to nurse on duty or D.O.N.
  • Assist residents with oxygen and other medical problems as needed.
  • Monitor health, safety, and well being of the residents.
  • Document information pertaining to changes in residents’ functional status, personal needs, appointments, etc.
  • Report concerns each shift about residents to nurse on duty and coworkers to provide for continuity of care.
  • Respond to resident emergencies; provide first-aid assistance and arrange for appropriate medical attention/ follow-up.
Other Job Duties
  • Encourage socialization among residents.
  • Assist in keeping the environment safe for residents.
  • When needed, provide back-up transportation services, using the company vehicle.
  • Identify supplies needed to perform tasks and report needs to supervisor.
  • Responsible for linen changes according to schedule and as needed.
  • Assist with resident laundry as needed.
  • Ensure general tidiness in resident rooms, to include trash removal and light clean-up.
  • Practice safety in working around and with equipment, chemicals, tools and utensils.
  • Assist with the activity program; conduct activities as directed by the Activity coordinator or assigned by supervisor.
  • Encourage and/ or remind residents to participate in activities. Promote activities enthusiastically to pique residents’ interest.
  • Chaperone activities outside the facility when requested.
  • Assist with serving meals and bussing tables during mealtimes.
  • Carry out other duties as assigned by supervisor.
Position Requirements
  • Training/ Experience as a nursing assistant or home-health aide
  • Ability to properly take temperature, pulse, respiration, weigh residents, and conduct an accurate blood pressure.
  • Skills in nurturing and organizing similar to those in a domestic household
  • Display a positive attitude towards, residents, families, staff, supervisor and administrator.
  • Experience or interest in working with older adults
  • Ability to read, write & speak fluent English while on duty
  • High school diploma or GED, with reading, comprehension and writing ability at 12th grade level or higher
  • Maintain current training and certifications as deemed necessary by state regulations, safety council and other regulatory departments
  • Must meet the minimum standards of a Level II background check as determined by state regulations
Physical Requirements
  • Must be physically fit to safely navigate an 8-hour shift of standing, walking, lifting, bending, stooping, kneeling, pushing and pulling up to 100% of the shift.
  • This entails the ability to:
    1. Stand 100% of the time
    2. Walk 80% of the time
    3. Assist in lifting resident/materials up to 50lbs 60% of the time
    4. Assist in lifting resident up to 75lbs 20% of the time (using safety equipment)
    5. Assist in lifting resident up to 100lbs 5% of the time (only when using safety equipment and additional staff)
    6. Bend 60% of the time
    7. Squat 25% of the time
    8. Kneel 10% of the time
    9. Push up to 100lbs 15% of the time
    10. Pull up to 100lbs 15% of the time
    11. Sit 5% of the time
