Job Summary
A rehabilitation hospital client based in Chicago, IL has an onsite contract opportunity for a _ Licensed Social Worker _. Shift schedule is Monday thru Friday 9:00 AM to 5:30 PM; no after hours, weekends or on-call. This is a 13 week contract with possibility of extension.
The _ Licensed Social Worker _ is an active part of the multidisciplinary team and is focused on discharge planning and coordinating post acute services for the patients. _ Licensed Social Worker _ will ensure seamless transitions for patients when discharged back into the community; collaborates closely with Director of Social Work & Discharge Planning in addressing the emotional, social, and practical needs of patients and helping patients and their families access resources they need for a fulfilling stay and safe discharge.
Job Qualification
Required - Bachelor's in Social Work
Required - IL State LSW
Required - Minimum of 1 years experience in a hospital or rehabilitation Social Work role
Required - Minimum of 1 years experience care planning and discharge planning for TBI and NTBI patients
Required - Epic electronic medical record
