Mary Lee Foundation Rehabilitation Center is Hiring a Lead QIDP Near Austin, TX

Purpose: Mary Lee Foundation is a 501(c)(3) nonprofit organization dedicated to providing diverse services and housing to adults with various types of disabilities. We are looking for a Lead Qualified Intellectual Disabilities Professional to enhance the quality of patient management, maximize satisfaction and promote cost effectiveness. The successful candidate will be accountable for the full patient care cycle. Duties include assessing, planning, implementing, monitoring and evaluating actions required to meet the client’s health and human services needed.

Responsibilities:

  • Fulfill all standard MLF QIDP responsibilities including:
  • Coordinates admissions and discharges of residents and prepares all appropriate documentation.
  • Facilitate the interdisciplinary team (IDT) process in conjunction with all residents assigned to caseload.
  • Facilitate the annual staffing of individual residents to develop and monitor treatment plans and Individual Program Plans (IPPs).
  • Is aware of available resources in the Austin community and provides information and makes referrals to specific services as needs of the client and family indicate.
  • Stay abreast of current best practice to provide optimal support for clients.
  • Facilitates family involvement and encourages communication between family and resident.
  • Performs necessary documentation, record keeping, report writing and any correspondence in required timely manner and in accordance with established procedures and incompliance with state standards such as but, not limited to; annual staffing reports, IPPs, IDRCs, ICAPS and monitoring of progress notes.
  • Develop goals and objectives for a resident’s IPP, monitor all areas of training, and enforce any IDT recommendations as determined in the annual staffing by the IDT.
  • Coordinates with nurse to ensure that all client medical needs are taken care of in a timely manner.
  • Works direct care shifts as needed when on call. Must be able and willing to perform all duties on the Direct Care Staff Job Description.
  • Adhere to professional standards as outlined by protocols, rules and regulations.
  • On call one full week every four weeks.
  • Fill in overseeing or Directing ICF, Transitional and Daybreak program operations as needed.
  • Assist all programs during State surveys to ensure compliance.
  • Provide information to the Central Office Accounting Department and MLF Board as required.
  • Be available at all times and be present for assigned shifts including carrying a cell phone and being flexible in emergency situations (On-Call).
  • Exhibit professional demeanor in matters of language, dress and behavior.
  • Follow and model agency policies and procedures.
  • Oversee and /or conduct all required record keeping and filing.
  • Cooperate and communicate well with Supervisor and MLF Executive Director
  • Meet on a regular basis with Director of ICF
  • Use time efficiently and delegate responsibility as needed.
  • Set clear priorities and follow through on them.
  • Performs complex, detailed, and specialized tasks to understand HHSC guidelines accurately. This includes auditing client files, attending training, and reviewing billing guidelines to ensure compliance with HHSC's standards and the Texas Administrative Code.

Supervisory Duties:

  • Create a positive, safe and productive work environment for all staff.
  • Supervise and direct personal including: counseling, disciplining, and terminating personnel to maintain and ensure an adequate staff of qualified personnel at all times.
  • Teach and model for staff the philosophy of independent living skills and to ensure that staff learn ways of teaching residents these skills.
  • Enable open communication with staff and to encourage staff to make recommendations regarding any improvements in the quality of residents’ lives.
  • Assist with staff training, to ensure that staff follow the daily schedule and perform assigned duties.
  • Observe and monitor staff especially during the 90-day probationary period.
  • Supervise ICF Case Managers and House Managers.
  • Need to provide Case manager training for onboarding and ongoing.
  • Assisting with ADP timesheet correction every pay period
  • Oversee the billing department to manage billing in TMHP and supervisee the use of EMR
  • Responsibilities are not limited to the above list and may include other duties as deemed necessary by supervisor.

Knowledge, Skills and Abilities:

  • Excellent knowledge of case management principles, healthcare management and reimbursement
  • Previous experience with psychological aspects of care
  • Effective communication skills
  • Excellent organizational and time management skills
  • Familiarity with professional and technical emerging knowledge
  • Problem solving skills and ability to multi-task
  • Compassionate with teamwork skills

Qualifications:

  • 2 years experience working with people with disabilities
  • Supervisory experience preferred.
  • Bachelor’s degree in a human services related field (i.e., social work, special education, psychology, rehabilitation counseling, or sociology)
  • Proven working experience in case management, including as a nurse, medical, mental health care manager or a related job
  • Clean criminal history and driving record with a valid driver’s license

With or without reasonable accommodation, the physical and mental requirements of this job may include the following: seeing, hearing, speaking, and writing clearly. Occasional reaching with hands and arms, stooping, kneeling, crouching, crawling, frequent sitting, standing and walking, may be required for long periods of time and may involve climbing stairs, walking up inclines, and on uneven terrain. Additional physical requirements may include, frequent lifting, bending down, and/or moving up to 50 pounds.
