Description of Duties
· Directs the planning, organization, development, and evaluation of program activities.
· Participates in annual grant preparation, including budget, objectives, narrative, and nutrition education plan.
· Provides direction, supervision, and evaluation of program staff.
· Assures that staff meet guidelines of USDA, IDPH and all other program agencies.
· Serves as resource person for in-service education and training program for MCH staff; orients and trains new staff.
· Analyzes and evaluates programs to insure and improve the quality of services offered.
· Monitors and evaluates monthly caseload statistics and implements corrective action as indicated.
· Assures maintenance of program activities records to meet State and Federal requirements.
· Assures that monthly reports are submitted to state and regional supervisors as required.
· Provides a monthly statistical report to administration on caseload management and program hours.
· Participated in assessment of program activities during state and federal program review/audits.
· Assures provision of nutrition education as required by State.
· Provides follow-up of program decisions by State.
· Oversees operation and functioning of state-wide program computer systems.
· Facilitates effective working relationships with a variety of community, governmental, and professional agencies.
· Oversees maintenance of program inventory.
· Communicates with Director of Client Services regarding program progress and needs.
· Performs other duties as assigned or requested.
Personal Characteristics
With minimum direction, assumes coordinating responsibilities. Communicates effectively with office and department personnel, clients, families, physicians, and community agencies. Is professional, self-controlled, and mature. Maintains strict confidentiality regarding client and office matters. Demonstrates organizational skills and ability to work independently.
Education and Training
Registered professional nurse licensed pursuant to Section 12 of the Illinois Nursing Act of 1987 (IL Rev. Stat. 1989, Ch. 111, par 3512) with a Bachelor’s Degree, preferably BSN, from a recognized or accredited program and one year of experience in community health or maternal and child health nursing. Knowledge of WIC and Family Case Management programs a plus.
