Under the supervision of the PACE Director, the RN Participant Care Manager provides QI oversight and fosters communication among multiple team members, and may provide medical and nursing care, ensuring high quality standards. They will focus on quality initiatives and auditing of electronic medical records and appropriate follow up.

Position Responsibilities:

  • Ensure Horizon PACE furnishes comprehensive medical, health, and social services that integrate acute and long-term care, including services furnished at the PACE center, the home, contracted, and inpatient facilities, in accordance with Horizon policy, State, and Federal regulations.
  • Monitor participant care to ensure services are provided as expeditiously as the participant's health condition requires, 24 hours a day, every day of the year, taking into account the participant's medical, physical, emotional, and social needs and per regulatory requirements.
  • Conduct continuous monitoring to ensure effectuation of services.
  • Document, track and monitor the provision of services across all care settings in order to ensure the interdisciplinary team remains alert to the participant's medical, physical, emotional, and social needs regardless of whether services are formally incorporated into the participant's plan of care.
  • Facilitating aspects of the treatment plan, such as care coordination and discussions with the family of the participant.
  • Develop effective working relations and cooperate with medical IDT throughout the entire case management process.
  • Facilitate communication among the IDT members.
  • Act as a liaison with contracted entities by facilitating communication and exchange of information between the PACE organization, PCP, IDT, and contacted and in-patient facilities, services, and specialists.
  • Ensure participant information is available and present to the IDT as expeditiously as needed to promote safe and efficient participant care.
  • Act as a member of the IDT, within their scope
  • Provide direct participant care, as needed.
  • Provide education activities and opportunities for the IDT, direct care staff, and contracted providers.
  • Acts within the scope of his or her authority to practice
  • Assists with monitoring of clinic department expenditures and meets budgetary expectations.
  • Follows all Horizon program policies and procedures and applicable Occupational Safety and Health Administration (OSHA) safety guidelines.
  • Protects privacy and maintains confidentiality of all company procedures, results and information about employees, participants and families.
  • Handles potentially infectious specimens with appropriate biohazard precautions, and practices Universal precautions.
  • Participates in continuing education classes and any required staff and training meetings. Maintains professional affiliations and any required certifications.
  • Participates in and supports Quality Improvement initiatives.
  • Performs other duties as required or requested.

  • Is knowledgeable of participant rights and ensures an atmosphere which allows for the privacy, dignity and well-being of all participants in a safe, secure environment.

  • Assist supervisor with various tasks to support department operations as directed; may also be assigned special projects.
  • Perform tasks which are supportive in nature to essential functions of the job, but which may be altered or re-designed depending on individual circumstances.
  • Assists participants and caregivers in filing grievances

Experience & Education:

  • Graduate of a school of professional nursing program required. Current and unrestricted Kentucky Registered Nurse License and Cardiopulmonary Resuscitation (CPR) certification required.
  • Preferred to have a minimum of one year’s experience in working with the frail or elderly population required.

Pay: $30.00 per hour
