The purpose of the Physical Therapist ( PT ) is to treat residents who have been referred to physical therapy by the resident's physician on written order. Identifying residents by diagnosis or observation or staff referral who may benefit from therapy and requests orders for evaluation and treatment. Forest Hill is an outpatient physical therapy facility dedicated to providing patient-centered care from highly skilled staff. • Able and Willing to work PRN, Per Diem

• Experience in a Skilled Nursing Facility Preferred
