Mental Health /Addiction Case Manager - Full Time Feinberg Consulting, Inc, a Joint Commission Accredited Organization with offices in Michigan and Florida, is seeking a Case Manager having experience with populations with neurobehavioral, psychiatric, substance abuse/misuse of all ages. The Case Manager’s role requires someone possessing strong critical thinking and interpersonal skills, highly organized and self-driven, works well with a multidisciplinary team to complete assessments, develop care plans and implement activities to maximize outcomes. Main Responsibilities:
Advocate, educate, and coordinate care with interdisciplinary team to maximize client outcomes.
Support with completing client assessments and development of care plans.
Identify client centered goals and work with clients to achieve outlined goals.
Collaborate with VP Clinical Services and other disciplines (Family Coach, Recovery Coach, Nutritionist, etc.) working with client and family in accomplishment of client and family goals.
Schedule and attend client appointments and coordinate all services from those appointments.
Coordinate behavioral health services, transportation, and other needs as outlined in plan of care.
Daily documentation through billing log of all Case Management activity and completion of initial and monthly client reports.
Compensation & Benefits: Competitive salary, health insurance benefits, Dental, Vision, 401(k), Life Insurance, Short Term/Long Term Disability, and PTO. Salary range between $65k-$74k annually.

Requirements:
Masters Mental Health or Masters Social Worker degree.
Prior experience working with substance abuse, mental health and neurobehavioral diagnoses of all ages.
Valid driver’s license, insurance, and reliable transportation for local travel within the Palm Beach County and surrounding areas.
Possesses strong ability to interface with technology (utilizing a smart phone, Microsoft Office, and web searches).
