As a pioneer in responsible fatherhood, family strengthening, and workforce development, CFUF’s programs seek to help clients fully exit poverty and sustain themselves and their families. CFUF serves Baltimore’s most marginalized populations living in high crime and high poverty communities. Our members (the term we use when referring to our clients) are 98% African-American, 60% male, 89% unemployed, 50% ex-offenders and homeless, and 30% without high school diplomas. Over 90% face one or multiple temporary or chronic personal, logistical, or human barriers.

All In is the Center for Urban Families’ comprehensive strategy to accelerate social and economic opportunity and advocate for policies that promote equity and racial justice. Leveraging 20 years of positive impact and applied learnings, the All In strategy targets two critical areas: chronic unemployment and family instability. The approach links individual accountability, person-centered case management, and supportive networks with workforce development training, education, and civic engagement. POSITION SUMMARY:

The All In Case Manager is a vital member of the CFUF team. All In Case Manager will provide intensive case management services for enrollees and members in the Clean Corps Program throughout the duration of their participation in the program. The intensive case management will include assistance with person-centered goal plan development, barrier removal support, administering surveys and assessments, coaching according to the EMPath model of case management’s guidelines and procedures. Work in collaboration with the Clean Corps project manager/trainer.

POSITION RESPONSIBILITIES:

· Track and manage caseload of CFUF members and program enrollees.

· Provide strength-based case management, and work with members to set goals and enhance their self-esteem and self-confidence, including the development of person-centered EMPath goal plans.

· Maintain case notes, paper and electronic files, and member databases, in real time, as specified by compliance standards.

· Participate in case conferencing to exchange information among case management staff about members to enhance their care coordination.

· Refer members to internal and external programs and services (e.g., housing, long-term counseling, child-care, substance abuse counseling, mental health counseling, diversion, and expungement services) that can assist them with removing barriers and resolving issues preventing them from achieving social and economic mobility.

· Coordinate with CFUF Employment Specialists to assess members' successes and challenges at work or during training.

· Work with other project manager/trainer to ensure members’ success in completing training programs as well as obtaining employment.

· Upon completion of the Clean Corps program in coordination with Employment Specialists, match members with open position(s) according to experience and skills.

· Prepare members for one-on-one and group interviews, job fairs, phone interviews, and computer assessments.

· Present information using a variety of instructional techniques and formats, such as role-playing, tool building exercises, simulations, team exercises, group discussions, videos, and lectures.

· Evaluate the progress of enrollees within training programs making individual member assessments and supportive service recommendations.

· Assist members with career exploration by utilizing skills and career assessment tools.

· Assist with the development of modules and implementation of projects affiliated with the Clean Corps program model.

· Support outreach efforts and develop community partnerships to include neighborhood outreach and presentations to community partner organizations.

· Participate in scheduled case audits.

· Conduct assessments and track results for the Bridge to Self-Sufficiency.

· Work outside of standard business hours, occasionally.

· Stay current on new methodologies and practices in the case management and transitional employment fields.

· Communicate with Clean Corps staff on the progress of enrollees.

· Participate in outreach/marketing activities to garner interests for the CFUF/ Clean Corps initiative.

· Conduct site visits as needed to Clean Corps sites.

· Participate in supervision meetings.

· Complete other tasks as assigned or needed.

KNOWLEDGE, SKILLS & ABILITIES:

· Bachelor's degree in human services or related field strongly preferred.

· At least 3 years of experience working in case management/family support area or related field.

· Demonstrated ability to build relationships, establish trust, and engage individuals of diverse educational and socio-economic backgrounds.

· Substantial understanding of CFUF’s current interventions: workforce development, responsible fatherhood, family services, and supportive services for disadvantaged individuals.

· Knowledge and experience with service agencies and resources for disadvantaged individuals required.

· Knowledge of transitional employment best practices.

· Must have experience with direct service delivery.

· Ability to work in a fast-paced team environment required.

· Must have excellent time management, organizational and communication skills.

· Ability and willingness to travel and attend meetings and/or appointments required.

· Proficient technology skills including Word, PowerPoint, Outlook, and data management systems.

· Must be resourceful, resilient, and tenacious with a high level of personal integrity and passion for CFUF’s mission to accelerate social and economic opportunities, and advocate for policies that promote equity and racial justice.
