Ativo Senior Living of Prescott Valley is Hiring a CARE PARTNER (MC) - Full Time Near Prescott, AZ

The Care Partner is an active team member and participant in creating connections in the lives of the resident. The Care Partner is responsible for the delivery of services including activities of daily living such as: assistance with dressing, bathing, and grooming. This is a critical position for residents who have extended trust to our community, therefore, it is important that the Care Partner is passionate about respecting residents and honoring resident rights. The Care Partner should be familiar with the resident, their story, and their care plan.
Essential Responsibilities:
1. Follow the service/care plan for each individual resident safely providing the activities of daily living and other services in a way that is respectful and maintains dignity for the resident.
2. Assist in serving meals, assists residents with eating, if necessary, and monitoring the dining room.
3. Perform housekeeping duties with the resident if indicated on the service/care plan; otherwise perform the duties as necessary to maintain a safe, clean, and pleasant environment for each resident's apartment in the community.
4. Assist as needed with group and individual activities.
5. Appropriately utilize the electronic devices and software provided.
6. Chart required information as instructed, including any resident changes. Add appropriate residents to Alert charting.
7. Follow infection control procedures, as established by the community.
8. Report all incidents/accidents to Health Services Director promptly.
9. Continue education and training requirements by deadline.
10. Work collaboratively with team members in and outside of the Health Services department.
11. Maintain privacy; HIPAA.
Physical Requirement:
Ability to walk throughout community continually during the shift. Push and pull wheelchairs and other equipment up to 300 lbs. Physical ability to assist residents. Bending and kneeling. Ability to use handheld device.

Food Handlers Certification
Fingerprint Clearance
