JOB DESCRIPTION

We are looking for a compassionate and dedicated Care Coordinator to join our team. This role involves scheduling and overseeing the delivery of care to our clients, coordinating with caregivers, and acting as a liaison between clients, their families, and healthcare professionals. QUALIFICATIONS

  • Experience in a coordination or administrative role, preferably within a healthcare or caregiving environment.
  • Strong organizational skills and the ability to manage multiple tasks simultaneously.
  • Excellent communication skills, both written and verbal.
  • A deep sense of empathy and a commitment to delivering client-centered care.
  • Ability to work effectively both as part of a team and independently.
  • Familiarity with MS Office and any relevant care management systems.
  • While not required, previous experience in home care is an advantage.

RESPONSIBILITIES

Schedule Management: Organize and manage caregiver schedules to ensure consistent and reliable care delivery for all clients. Adapt schedules to accommodate unexpected changes or emergencies to maintain continuous support.

Client and Family Liaison: Act as the primary point of contact for clients and their families. Address inquiries, provide updates, and facilitate open lines of communication regarding the care process. Ensure a high level of client and family satisfaction through proactive communication and swift resolution of any concerns.

Caregiver Coordination: Coordinate daily logistics for the caregiving team, ensuring that all caregivers have the necessary information and resources to provide outstanding care. This includes briefing caregivers on specific client needs, health status, and special requests. Provide ongoing support to the caregiving team to ensure they are equipped with the latest best practices in home care.

Quality Assurance: Conduct regular check-ins and follow-ups with clients and caregivers to assess the quality of care delivered. Implement quality improvement measures to enhance service delivery and client satisfaction.

Documentation and Record Keeping: Maintain accurate and detailed records of all client and caregiver interactions, and any incidents to ensure compliance with healthcare regulations and agency standards.
