Position: Physical Therapy Assistant - Home Care

Location: 11226, 11225, 11217 BROOKLYN, NY

Requirements:

· must have completed an accredited Physical Therapist Assistant program and an associate degree program.

· must have passed the National Physical Therapy Exam for Physical Therapist Assistants (NPTE-PTA)

· Must have CPR Certification

· must-have will participate in practical experiences to gain hands-on training in providing physical therapy services.

· must have reliable transportation is often necessary to reach patients' homes.

· must have ethical standards and professional conduct set by the American Physical Therapy Association (APTA)

Responsibilities:

· Collaborate with the supervising Physical Therapist to assess patients' physical conditions, mobility, and limitations in their home environment.

· Observe and document changes in the health status and living conditions.

· Assist the Physical Therapist in developing individualized treatment plans tailored to the patient's specific needs and goals.

· Educate patients and their caregivers about exercises and activities designed to improve their mobility and overall physical health.

· Conduct home safety evaluations to identify potential hazards and make recommendations for modifications or equipment that can enhance patient safety and mobility.

· Maintain accurate and comprehensive patient records, including progress notes and treatment plans.

· Collaborate with other healthcare professionals involved in the patient's care, including physicians, nurses, and social workers.

· Adhere to all healthcare regulations and privacy laws (e.g., HIPAA) in a patient's home

· Travel to patients' homes and manage your schedule efficiently to provide care to multiple patients in different locations.
