As a Case Manager RN you will oversee and help manage the delivery of home-based care from admission to discharge. You will help ensure clients and their families receive appropriate utilization of services and coordination of care.

Must have:

• Successfully graduated from an approved and certified RN program
• Current and unencumbered license to practice as an RN in the state
• Ability to make quick judgements and work independently
• Must be knowledgeable of home health nursing practices, procedures, and guidelines that pertain to home health care.

Nice to have:

• 1 years’ experience as an Registered Nurse (RN)
• Active patient or bedside care within the past year
