We are looking for a Licensed Vocational Nurse to join our home health team. The ideal candidate will have a current LVN license and experience in providing home health care services. The successful candidate will be responsible for providing direct patient care, assessing patient needs, developing and implementing nursing care plans, and educating patients and families on disease management. The LVN will also be responsible for administering medications, treatments, and therapies as prescribed by the physician.

Responsibilities:

• Provide basic patient care under the direction of a registered nurse or physician.
• Assist patients with activities of daily living, such as bathing, dressing, and eating.
• Observe and record patient's vital signs and other health information.
• Administer medications and treatments as prescribed by a physician.
• Provide home health services, such as wound care and catheter care.
• Assist in the development of individualized patient care plans.

Job Types: Part-time, PRN, Per diem

Pay: $35.00 - $40.00 per hour
