The Community Action Team (CAT) provides in-home family and individual case management, therapeutic mentorship, parental support, psychiatrist and nursing care plus medication management to youths ages 11-21 with a mental health or co-occurring order diagnosis. Services are up to 6-9 months or more. The Therapeutic Mentor will be responsible for providing advocacy, training of developmental life assets, and mentoring services to children/adolescents in Hillsborough County within the home and school environment. Participates as a member of a multidisciplinary treatment team. Reports to the unit/department supervisor.
REQUIRED QUALIFICATIONS :

• Associates degree in Human Services related field or high school graduate with one (1) year experience with adolescents in a clinical or recreational setting with Children.
• Must be, at minimum, 21 years old with a valid Florida Drivers License and reliable transportation/
• Computer efficient to navigate through various programs and strong communication skills.

FUNCTIONAL RESPONSIBILITIES INCLUDE:

• Provide therapeutic mentoring/respite services in a timely and reliable manner in accordance with the policies, procedures, and applicable provisional standards.
• Complete all documentation based on established timelines as per program requirements.
• Provides responsible activities with consistent supervision to clients and their families.
• Keep direct supervisor and treatment team informed at all times of relevant consumer and family issues.
• Maintain efficient workflow management and organization as per program requirements.
• Meets with case manager and family to establish specific details for implementing home and community based rehabilitative and respite services.
• Other duties as assigned or needed as per program requirements.

REQUIRED COMPETENCIES

• Ability to read, analyze, and coordinate with multidisciplinary team regarding procedures, and governmental regulations.
• Ability to write reports and document mentoring services and effectively communicate verbally with, provide information to, and respond to questions from managers, patients, service providers, managed care entities, family members, and other stakeholders.

PHYSICAL DEMANDS:
Abilities in reaching, bending, talking, fingering, sitting, carrying, standing, grasping, fine hand coordination, ability to hear, ability to read and write, and the ability to remain calm under stress. Ability to reach with hands and arms, climb stairs, balance, stoop, kneel, crouch or crawl. Specific vision abilities include close, distance, color, peripheral, depth perception and the ability to adjust focus. Must be capable of lifting 50 pounds of dead weight alone. May be required to complete standing presentations.
WORK ENVIRONMENT:
High-paced, collaborative team performance with the emphasis on critical thinking and decision-making with guidance from program supervisor/manager. Climate controlled office environments with frequent exposure to clients. Travel within Hillsborough County via personal vehicle required to facilitate in-home client services in a variety of socioeconomic neighborhood
