Essential Functions:
• Follow operation procedures and personnel policies.
• Review of all data. Log notes, event/med error reports, on-call logs, medication logs, and use information to provide effective services.
• Utilize Positive Behavior Supports Strategies and keep staff educated.
• Act as liaison in all areas of community life.
• Required on-call and crisis intervention as needed
• Communicate with Regional Center and Targeted Case Management regarding people and services provided.
• Maintain required documentation on all individuals, to include any and all electronic documentation.
• Submit information as required by supervisor.
• Maintain chain of command and communicate with supervisor.
• Ability to follow directions/instructions from supervisors.
• Remain up to date, attend, and conduct training sessions as needed.
• Attend management/staff meetings and serve on committees as needed.
• Responsible for aiding the individual in purchasing groceries, supplies and clothing and
with keeping a current inventory.
• Maintain a safe and clean environment.
• Monitor the individual’s physical and emotional health and ensure their needs are met.
• Ensure all medical needs are taken care of.
• Remain knowledgeable and relay/report individual’s physical, medical, and mental conditions to the appropriate parties.
• Ensure individual privacy is maintained in the home and in the community.
• Treat individuals with dignity and respect, as you would expect to be treated.
• Be an advocate for our individuals and serve as a role model.
• Promote and encourage independence in every individual.
• Provide opportunities to be involved in the community and activities, both private and public.
• Offer choices and encourage all individuals to indicate preferences.
• Ensure the individual is supported in an environment free of abuse, neglect and intimidations as defined by the Department of Mental Health.
• Provide an atmosphere and attitude conducive to a positive teaching experience.
• Report all incidents or suspicion of abuse or neglect to supervisor immediately.
• Provide for or arrange all transportation needs of the individual.
• If lifting is needed, use proper lifting techniques after proper training.
• Administer medication as needed after proper training.
• Alternate weekend/holiday on call coverage for all homes when required.
• Periodically adjust work schedule as necessary to meet the needs of the individuals needs or to meet the time off needs of other staff members.
• Work unusual or extra hours occasionally, as needed.
• Work in place of Direct Support Professionals as situations arise.
• Work in other “Easy Living” facilities when requested by management.
• Perform additional duties not specifically mentioned above when emergencies or unusual situations arise.
Supervisory Functions:
• Supervision of Direct Support Professionals
• Impose discipline for situations as specified in the personnel policies
• Attend staff meetings when specified by management
Administrative Functions:
• Education for the general public to better serve people with disabilities.
• Become knowledgeable of and implement quality assurance activities and certification standards, both for the individual services and for general agency function.
• Maintenance of positive relationships with families, guardians, and/or advocates for individuals of the facility.
• Possess a professional appearance when representing the agency.
• Maintenance of positive public relations.
• Establish positive working relationship with other staff employed in the company, as well as other providers and state agencies.
• Responsible and accountable for company credit card when in possession of.
• Participate in facility Quality Assurance (QA) review.
• Become certified as a CPR/First Aid/Mandt Instructor, as requested by Supervisor, and alternate with other instructors of the agency to conduct training sessions.
