Key Details:

  • Title: Registered Nurse (RN) Case Manager
  • Pay: $40 - $45 per hour based on experience
  • Schedule: Full-time; Monday – Friday; variable shifts between 8 am – 6:30 pm with the option of 5-8’s or 4-10’s.

Who You Are: You are a compassionate RN with exceptional communication skills and a dedication to improving outcomes for complex patients. You thrive in a team-oriented environment and excel in patient education, case management, and program coordination. You will also have:

Basic Qualifications:

  • Associate degree or higher in Nursing from an accredited program
  • Current Oregon RN license
  • At least 1 year of experience in a clinic setting as a nurse, or 2 years as a medical assistant
  • Current CPR certification
  • Working knowledge of state healthcare laws, licensing board regulations, and controlled substance regulations

Preferred:

  • Experience working in a community health clinic
  • Knowledge of data and chronic disease registries
  • Bilingual in Spanish preferred; all multilingual speakers highly encouraged to apply

Your Role: As an RN Case Manager, you'll work with new and existing patients in the development, implementation, and evaluation of individualized care plans for high-risk, complex patients. You'll advocate for patient safety and serve as a liaison between healthcare providers, community services, families, and patients themselves. Responsibilities Include:

Clinical Responsibilities:

  • Work closely with providers to enhance patient experience, understanding, and involvement in their own healthcare
  • Provide triage and advice for all types of patients per licensing, standing orders, and protocols
  • Complete patient appointments following RN standard of practice and clinic policies
  • Actively participate in CareTeam including case management and meeting with clients to review treatment plans and follow-ups
  • Assist clients in establishing individualized health goals and provide education resources and advocacy
  • Review medication lists, changes to medication regimen, and prescription refills per standing orders
  • Maintain good working relationships with community health partners and coordinate with grant and program managers

Wallace Team Member:

  • Nurture healthy working relationships with staff, volunteers, and patients
  • Maintain strict patient, staff, and organizational confidentiality
  • Demonstrate a deep concern for the health and well-being of others
  • Participate in compliance, quality assurance, and quality improvement activities as directed
  • Additional responsibilities as assigned
