Position: SUD Outreach Worker

Region: North / East County

The SUD Outreach Worker reports directly to the program manager. H/she is primarily responsible for providing those disproportionately impacted by substance use and who have traditionally lacked access to care, including homeless, near homeless, or other vulnerable individuals with primary substance use conditions with appropriate referrals and linkages to needed resources, including substance use disorder and co-occurring mental health treatment as well as supportive housing. Provide overdose prevention education and deploy evidence-based strategies to lessen the harms associated with substance use. Develop and maintain positive relationships with community members and referral agencies in appropriate outreach region(s) with special focus on underserved communities. Administer screenings and conduct assessments within scope of practice to identify individualized needs and to address barriers.

Qualifications, Skills, Knowledge, and Experience: Must have two years’ experience in behavioral health services or one year of experience and a bachelor’s degree in a human services field. Must have a clean driving record, with a minimum of two years’ driving experience. Must be free of probation or parole for at least one year. Past experience should demonstrate success working vulnerable individuals and providing case management; strong knowledge of and relationships with community resources; understanding of engagement, outreach, and motivational interviewing techniques; ability to maintain up-to-date records, documentation, and filing systems; strong knowledge of the needs adults with substance use disorders and the systems that serve them; and proven success with multidisciplinary teams and a spirit of collaboration and innovation. Must comply with all agency and contract requirements related to trainings, public health certifications, reporting, etc. Must be able to pass drug test, background clearance, and provide negative TB test. Bilingual proficiency preferred.
