We are seeking a compassionate and skilled Registered Nurse to join our Home Health team. As a Home Health RN, you will provide high-quality care to patients in the comfort of their own homes. Your primary responsibility will be to assess patient needs, develop care plans, and coordinate with other healthcare professionals to ensure the best possible outcomes for our patients.

Responsibilities:

• Conduct comprehensive assessments of patients' health status and develop individualized care plans.
• Conducts initial and ongoing comprehensive assessments of the client's needs, including Outcome and Assessment Information Set (OASIS) assessments.
• Performs starts-of-care, resumptions-of-care, re-certifications, and discharges.
• Respond to Emergency situations.
• Provide wound care, ostomy care, and manage catheters
• Collaborate with physicians, therapists, and other healthcare professionals to coordinate patient care.
• Ensures that required documentation is complete in a timely manner and is in compliance with regulations and standards.

Requirements:

• Valid RN license in the state of Florida.
• Minimum of 2 years of nursing experience, preferably in home health or a related field.
• Valid Drivers License/Auto Insurance.
• Current CPR Card.
• Experience with durable medical equipment (DME) and its management.
• Proficient in conducting physical assessments and interpreting diagnostic tests.

Pay: $37.98 - $45.75 per hour
