Summary

The Substance Abuse Counselor is responsible for providing comprehensive substance use counseling services to people with a history of addiction and providing specialized support to clinic providers and MAT Hub and Spoke staff regarding people suffering from or suspected of substance abuse.

Essential Functions

  • Performs all job functions in a professional and courteous manner. This includes answering all general phone call timely. Provide excellent customer service to internal and external patients by being responsive to all inquiries in a timely manner.
  • Performs job duties collaboratively with health center management and exercises good judgement.
  • Fosters and promotes a culture of service excellence and accountability.
  • Consistently adheres to a high standard of professional ethics; conducts self in an ethical manner and is a role model to others.
  • Complies with organizational policies, procedures, and meeting all annual credential requirements.
  • Perform triage services, crisis intervention within scope of practice, case management, referrals and follow-up as required
  • Provide SUD services to a minimum of 7 patients per day and completing all EHR required documentation on a timely manner.
  • Developed individualized treatment plans for patients associated with ASAM criteria, maintain patient’s records, monitored progress toward treatment goals, and conducted annual reviews of active treatment plans
  • Accepts newly assigned cases in which a licensed provider has completed the diagnostic assessment. Completes case management assessment on all patients assigned to case manager
  • Assist with all documentation of services provided for contract compliance and Quality Assurance (QA) and provide statistical/reports as assigned.
  • Provides on-going supportive and/or case management functions in accordance with the problems, needs, and the strategies identified within the service plan to assist the patients achieve the stated goals and objectives.
  • Maintain communication and collaboration with Tarzana treatment Clinic, AV Hospital, and Detox programs regarding client referrals and provision of services.
  • Facilitate SUD / AOD support groups as needed.
  • Provides ongoing face to face reviews with the patients on a regular basis to assess the progress made in reaching service goals so that the service plan can be modified as necessary to ensure that the goals and objectives are being achieved.
  • Documents all service contacts within 24 hours of contact including face-to-face, collateral and networking contacts. Demonstrate appropriate & complete documentation of treatment – related information including treatment plans and reviews, progress notes, correspondence, releases of information, etc.
  • Interfaced and maintained a close working relationship with a variety of intra- and inter-agency multi-disciplinary professionals and organizations including on-staff psychiatrists and medical professionals to collaborate regarding ways to provide well-rounded treatment services that target numerous areas of need for patients.
  • Participates in regular interdisciplinary staff meetings, doctor staff meetings and Substance Use Program supervision
  • Participates in regular supervision as a means of enhancing professional growth, reviewing the provision of clinical services, and addressing administrative issues.
  • Maintains collaborative relationships and communication with community referring agencies to formulate appropriate collaborative intervention strategies.
  • Performs other duties as assigned by the supervisor, which are consistent with the position and in compliance with agency policies and procedures.
  • Evaluate for immediate needs of crisis intervention, inpatient detox, medical attention, showers, clothing, food, emergency shelter, etc. Assure access to local resources by developing and maintaining relationships with these organizations.
  • Maintain documentation of all counseling sessions, substance abuse services and other services in compliance with agency policies and procedures, contracts, and other regulatory requirements.
  • As needed, provide services in the field on them mobile BH/SUD unit.
  • Other duties as assigned.

Physical Demands - This is mostly in an office setting; however, this position may require sitting for prolonged periods of time. Walking, bending, and reaching may be required as well.

Position Type and Expected Hours of Work - This is a full-time position. This is a Full-time position. Days and hours of work are Monday through Friday, hours range from 7:30am-6:30pm with occasional approved overtime.

Travel - On occasion travel will be required.

Education and Experience

  • Considerable knowledge of substance use disorders and DSM 5 disorders of adulthood. Knowledge of utilizing assessments in treatment planning for case management. Knowledge of community referring agencies and collaborative intervention strategies.
  • Demonstrated ability to develop effective service plans and strategies to meet identified needs; to work effectively with community resources in coordinating services for patients; to work closely and effectively with patients, and other professionals; to gather and document necessary patient information, write reports and maintain patient records; to effectively communicate both verbally and in writing, and to maintain confidentiality according to all relevant regulations and agency requirements.

Qualifications:

  • Must hold a CATC, CADC, CADAC, or CCAPP.
  • First-aid and CPR up to date
  • Current TB test results Required
  • AOD Counseling Certification: 2 years
  • Bachelor’s Degree in Psychology, Human Development, and/or Sociology preferred.
  • Valid Driver License
