Cross Road Health Ministries, Inc. is seeking a Licensed Clinical Social Worker (LCSW) licensed in the State of Alaska or ability to obtain State Licenses. This position can be either at our location in Glennallen or Delta Junction, Alaska. Position SummaryThis full-time/part-time and/or remote position requires a strong team player and a self-starter. You will work in collaboration with the primary healthcare team providing behavioral healthcare within scope of practice. Provides screening, assessment, diagnoses, evaluation, crisis intervention, treatment, consultation, care coordination and case management, education, referral, advocacy and other related outpatient services. You will make clinical recommendations and refer to appropriate levels of care in a team-based setting.

Essential Functions
- Screen new clients identified at risk in the primary care setting to identify presenting problems, address urgent issues and/or provide immediate referral as indicated.
- Administer psychosocial assessments and crisis intervention.
- Provide individual brief interventions to adults, adolescents, and children experiencing biopsychosocial stressors.
- Provide direct clinical services including clinical evaluations and counseling using motivational interviewing and brief intervention methods.
- Develop and write record notes and evaluation reports.
- Assist with service coordination and follow-up procedures.
- Coordinate consultation and services to various community agencies and health services providers.
- Collaborate with clients, physicians, and other providers to develop and implement client-centered interventions and treatment plans.
- Re-assess clients and complete new treatment plans as indicated.
- Able to work a full-time flexible schedule that will include evenings and some weekends.
