About the Organization Transformation House is licensed by the Minnesota Department of Human Services, we are a 245G Substance Use Disorder Treatment Facility. We have three residential sites, in Anoka and Fridley, and we have two sober living facilities, in Champlin and Fridley.

Description Purpose of Position
This position is responsible for providing mental health services to our residential and outpatient clients, including 1:1, Diagnostic Assessments, Facilitating groups.
Essential Duties and Responsibilities .
- Must adhere to all policies & procedures while performing duties..
- Assist Alcohol and Drug Counselors in recognizing and treating mental disorders and psychosocial problems.
- Work with clinical team to treat and manage clients with chronic emotional and/or health problems efficiently and effectively.
- Participate in clinical staffing
- Assist in detection of at-risk patients and development of plans to prevent further psychological or physical deterioration.
- Evaluate client care plans with clinical team.
- Teach clients, families, and staff about care, prevention and treatment enhancement techniques.

Position Requirements Minimum Training and Experience Required to Perform Essential Job Functions
- Masters degree in Behavioral Sciences or related program.
- Current state licensure as a Psychiatric Nurse, Licensed Independent Clinical Social Worker (LICSW), Licensed Professional Clinical Counselor (LPCC), Licensed Marriage and Family Therapist (LMFT).
- Must know and understand the implication of Minnesota Statute 245G, and Minnesota Statutes 245A (Human Services Licensing Act-HSLA), and sections 626.556 (Maltreatment of Minors Act-MOMA), 626.557 (Vulnerable Adult Act-VAA), and 626.5572 (VAA definitions).
