JOB TITLE: Behavioral Health Clinician- College Campus

SUPERVISED BY: Chief of Operations

STATUS: Hourly/Full-Time – Non-Exempt

Salary: $90,000 - $110,000

Position Purpose

Under direct employment with Via Care Community Health Center, the full-time position is at a College campus . The Behavioral Health Clinician will be based at a campus is working with students to provide crisis intervention, brief counseling, case management, triage & linkages to social services, housing linkages and other services to assist in providing comprehensive services to ensure better health and educational outcomes.


ADDENDUM TO STUDENT HEALTH FACILITY AGREEMENT

The LCSW (Licensed Clinical Social Worker) Counselor will monitor students and develop action plans to facilitate referrals to on-and off-campus resources. In addition, the LCSW Counselor will develop and lead programs, trainings, and workshops for faculty, staff, and students that support holistic student development and success.


Essential Duties & Responsibilities

  • Collaborates with ELAC Faculty, Staff & Administration to assist in screening and providing brief evaluation and crisis intervention as required.
  • Utilize students' existing resources plus identify and link with additional resources available through ELAC and the local community.
  • Conducts case management services to include comprehensive intake and assessment, implementation of care plan, subspecialty referrals, monitoring and follow-up on all referred students.
  • Develop, implement, monitor, and maintain a comprehensive care plan for students.
  • Contributes to program planning, evaluation, report preparation and development of articles.
  • Maintains, provides, and coordinates appropriate referrals for students at ELAC including referral and liaison with housing agencies, social services, mental health services, financial support agencies, food services, and other basic needs resources for students.
  • Collects and enters necessary data for reporting purposes, completes and submits monthly project activity and other reports as required by ELAC to ensure compliance with federal, state and local guidelines.
  • Participates in weekly case conferences with ELAC stat to help with problem solving related to patients and ensure professional guidance and high-quality case management services are provided
  • Supervises and trains 3-4 interns and unlicensed staff.
  • Develop and maintain a database of referral resources for social service agencies within the local community.
  • Participates as a member of the Behavior Intervention Team (BIT).
  • Develop an individual-based and/or group-based treatment plan, including medical, psychological, financial, and other life issues
  • Participation in Via Care Community Health Center's Continuing Quality Improvement (Ql) program.
  • Maintain confidentiality at all times
  • Performs other duties as assigned


Requirements

  • Master's Degree in Social Work with at least 1-year experience working in school-based setting providing crisis intervention, substance use disorder support, housing placements, general case management and counseling services.
  • Licensed as a Clinical Social Worker
  • Experience as case manager in health or community-based social services setting working with clients who have social, psychological problems, chronic medical conditions, homelessness or at risk of homelessness and/or young adults with serious mental illness or substance use disorders.
  • Data entry skills required. Proficient in utilizing Microsoft Word, Excel and Power Point.
  • Effective communication and customer service skills.
  • Strong desire to work as part of multi-disciplinary team including school staff, counselors, LCSW and another medical provider
  • Bilingual strongly preferred but required.
  • MSW from graduate school for Social Work
  • Licensed in the State of California by the Board of Behavioral Science Examiners
  • Demonstrated expertise/skills in working with the target population
  • Must be able to travel to off site and satellite locations
  • Must have insured personal vehicle
  • Handles emergency or crisis situations
