PACE helps seniors who need nursing home-level care maintain their independence by providing all the medical care and support services they need to continue living comfortably and safely at home.PRNPosition Summary:The PACE Speech Language Pathologist (SLP) is responsible for providing a comprehensive speech-language pathology services for participants including development and oversight of functional maintenance programs.The SLP evaluates and assesses participants regarding functional abilities including physical, emotional, cognitive, and sensory components to determine necessary interventions. The SLP plans , implements and documents individualized therapy in accordance with the interdisciplinary plan of care, policies and expectations of the PACE program according to regulatory standards.Position Duties & Responsibilities:Provides a comprehensive speech therapy program for participants including development and oversight of functional maintenance programs.Evaluates and assesses participants to determine functional abilities including physical, emotional cognitive and sensory components to determine necessary interventions.Assists participants in learning or re-learning tasks necessary to support activities of daily living (ADLs).Participates as needed as a member of the Interdisciplinary Team (IDT) r esponsible for speech evaluations and development and implementation of the individualized speech therapy plan of care (POC) in accordance with PACE policies, procedures, expectations, and regulations.Reviews and revises the POC in coordination with the IDT and per regulatory requirements as participant status changes and as driven by needs identified as significant by the participant/family/caregiver.Provides and recommends reeducation, therapeutic activity, therapeutic exercise, manual therapy, self-care/home management training, cognitive skills development, and other therapeutic interventions as necessary.Plans and develops individual and group exercise programs for participants.Communicates clearly, concisely and in a timely and positive manner with team members, community partners, referral sources and participants/families/caregivers.Displays ability to interact professionally and respectfully with geriatric individuals including those with cognitive decline and/or physical frailties.Observes each participant for changes in physical, mental, emotional, and social functioning and reports such changes to the IDT. Works with interdisciplinary teams to assure effective, cost-efficient care.Identifies and provides for educational needs of the participant/family/caregiver.Participates in center quality improvement activities.Functions independently, working in multiple settings including the PACE center, participant homes, long term care and assistant living facilities.Assists participants and caregivers in filing grievances.Completes documentation accurately and in a timely manner.Demonstrates knowledge of PACE criteria and is able to communicate the PACE philosophy in providing information to participants, families, caregivers, referral sources, providers and the public.Other projects and duties as assigned.Position Qualifications:Current license to practice as a Speech Language Pathologist in Kentucky.Minimum 1 year of experience working with a frail or elderly population or must receive appropriate training upon hire.Must maintain CPR certification.Must have the flexibility to travel within service area and provide infrequent care in participants home.Proficiency with technology-based tools, including electronic medical record.Able to work independently and as an effective member of a team.Excellent communication skills, with ability to communicate with internal stakeholders at all levels of the organization and external stakeholders from varied backgrounds.Excellent organizational skills; able to multitask and maintain numerous initiatives with time sensitivity.Working knowledge of principles and practices of healthcare, healthcare systems, managed care, Medicare and Medicaid benefits and regulations.Knowledge of safety and infection control requirements for healthcare facilities.Care Guide Partners PACE, 109 Buffalo Creek Drive, Elizabethtown, Kentucky, United States of America#J-18808-Ljbffr
