Sentinel Home Health Care is seeking a Physical Therapy Assistant to join our dynamic team, providing care in the following Counties: Mecklenburg, Iredell, Cabarrus, Union, and Rowan.

As a Physical Therapy Assistant with our agency, you will :
- Work with Sentinel participants in a home care setting.
- Perform therapeutic procedures by administering manual exercises.
- Contribute to physical therapist's effectiveness by identifying patient care issues.
- Document patient care services by charting in the patient records.
- Actively function as a member of an Interdisciplinary team and participate in case conferences.
- Provide continuity of care by implementing patient management plans under the supervision of a physical therapist.
- Instruct and counsel participants by describing therapeutic regimens, wellness, and health maintenance.
Job Qualifications:
- Be a licensed Physical Therapy Assistant in the state of North Carolina.
- An Associate in Applied Science in Physical Therapy Assistant
- Have at least one year of experience working with the frail and elderly population.
- Demonstrate clinical competency with the geriatric population.
- Have a valid NC Driver’s license and a dependable vehicle.
