Hale O Meleana in Honolulu, Hawaii is looking for a Part-time Licensed Practical Nurse to help us with sourcing, locating, and recruiting talented, best qualified, and culture-add candidates who embrace Ohana's mission and values. Under the general direction of the Director of Nursing, provides professional nursing health to patients following established policies and procedures of the agency. Provides services under the direct supervision of the Home Health Case Manager, providing direct nursing to the patient in the patient's place of residence. Answers questions from patients, responsible parties and other personnel. Performs job duties in accordance with Ohana Pacific Management Company's Mission, Vision, Core Values and Core Behaviors.

Shift: Part-time NOC Shift (10:00 pm - 6:30 am Saturday's and Sunday's)

Duties include but are not limited:

  • Demonstrates knowledge of the patient's diagnoses, limitations, expectations, plan of health, current status.
  • Takes verbal/telephone orders from physicians and transcribes orders to patient chart, treatment/health plans as appropriate. Follows-up on physician orders.
  • Administers treatments as ordered by the physician and according to policy and procedures.
  • Conducts accurate physical assessments of the patient, determines their needs and implements appropriate interventions to meet those needs.
  • Appraises the family, physician and others of changes/incidents.
  • Ensures infection control processes and procedures are followed by all staff in the home.
  • Orients and supervises new staff as assigned.
  • Collaborates with physicians, therapy services, consultants and agencies to improve quality of services and to resolve identified problems.

Requirements:

  • Minimum of 1 year of recent experience in general, medical/surgical nursing. Knowledge of medical terminology and abbreviation required.
  • Graduate of an accredited school of nursing.
  • Current license as a Registered Nurse in the State of Hawaii required.
  • Evidence of continuing nursing education and competency in basic nursing procedures.
  • Knowledge of state and federal regulations governing home health services preferred.
  • First Aid and CPR preferred. (can be done in-house).
  • Must have current medical/health screening including 2 step TB clearance (can be done in-house).
