Position Summary: As a Physical Therapist in our long-term care facility located in Glasgow, WV, you will be responsible for providing high-quality rehabilitative care to our residents to enhance their mobility, strength, and overall functional independence. Your role will involve conducting assessments, developing individualized treatment plans, and implementing evidence-based interventions to optimize residents' physical function and quality of life.
Key Responsibilities:
Conduct comprehensive evaluations of residents' physical function, mobility, balance, and strength to identify impairments, functional limitations, and rehabilitation needs.
Develop personalized treatment plans based on assessment findings, resident goals, and clinical reasoning, utilizing a variety of therapeutic modalities and techniques.
Implement therapeutic exercises, manual therapy, gait training, balance activities, and other interventions to improve residents' range of motion, muscle strength, endurance, and coordination.
Provide hands-on assistance and guidance to residents during therapeutic activities and functional tasks to promote safety, independence, and optimal recovery.
Monitor residents' progress, adjust treatment plans as needed, and document treatment sessions accurately and in a timely manner, adhering to regulatory standards and facility policies.
Collaborate with interdisciplinary team members, including occupational therapists, speech therapists, nurses, social workers, and physicians, to ensure coordinated and holistic care for residents.
Educate residents, families, and caregivers on therapeutic exercises, home exercise programs, fall prevention strategies, and adaptive equipment to support ongoing rehabilitation and maintenance of gains.
Participate in facility meetings, case conferences, and quality improvement initiatives to enhance clinical outcomes, promote best practices, and contribute to the overall excellence of resident care.
Qualifications:
Master's or Doctorate degree in Physical Therapy from an accredited program.
Current state licensure or eligibility for licensure as a Physical Therapist in West Virginia.
CPR certification and compliance with continuing education requirements.
Previous experience in long-term care, geriatric rehabilitation, or skilled nursing preferred.
Strong clinical skills, critical thinking abilities, and a compassionate approach to patient care.
Excellent communication and interpersonal skills, with the ability to collaborate effectively with residents, families, and interdisciplinary team members.
