Flexible Schedule
Hybrid Schedule-Hybrid is defined as a flexible work model that supports a blend of in the physical office (1-2 days per week) and partially remote workplace. Candidates must have the ability to flex their time to accommodate weekend and evening events as needed.
JOB PURPOSE
The Community Diabetes Initiative is a collaboration with HealthCare Access Maryland (HCAM) and the Baltimore City Health Department (BCHD) to reduce the risk of type 2 diabetes and to address the racial disparity of severity and prevalence of the disease for Baltimore City residents. The Community Diabetes Initiative is funded by a 5-year grant supported by the Center for Disease Control (CDC).
The Community Health Worker will outreach, screen, and enroll residents into resources including diabetes education, food, housing transportation and other benefits associated with diabetes prevention and management. The CHWs will plan and attend neighborhood outreach events, collaborate with community organizations to provide educational materials, and make referrals to Diabetes Prevention programs.
The Community Health Worker will serve as a bridge for Baltimore City residents, medical providers, and Diabetes Prevention Programs to increase awareness and knowledge, and to address Social Determinants of Health with a goal of providing early intervention or prevention of type 2 diabetes.

While performing the duties of this job, the employee is regularly required to talk, hear, and respond to customers and employees. The employee frequently is required to stand; use hands to finger, handle, or feel; reach with hands and arms; climb or balance; and stoop, kneel, crouch, or crawl. The employee is occasionally required to walk and sit. The employee must regularly lift and/or move up to 25 pounds and occasionally lift and/or move up to 50 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.

WORK ENVIRONMENT
While performing the duties of this job, the employee is exposed to both indoor and outdoor conditions. The employee will be subject to noise.
