The Care & Connect Mobile Nurse contributes to the calling of Medical Teams International by providing crucial nursing services for Medical Team’s Care and Connect mobile health clinics. The Registered Nurse will play a key role in conducting client assessments and screening, will utilize their nursing judgement and screening tools to determine follow up care and referral needs for the client, will administer direct nursing care when needed and may be called on to assist with client referrals, care coordination and data management.

ESSENTIAL DUTIES AND RESPONSIBILITIES

• Provide nursing assessment, planning and care coordination as needed.
• Provide direct nursing care as needed to patients at Medical Team’s mobile Care and Connect clinics.
• Administer care in accordance with Medical Team’s Health Guidelines, Standing Orders and evidence-based clinical practices.
• Apply Medical Team’s validated health screening tools.
• Ensure all patient care is accurately documented in the patient’s record.
• Establish partnerships with patients and families for positive outcomes in health status; identify patient goals for improving health status and management of chronic physical and mental health conditions; refer and connect patients to FQHCs and other community support services as needed.
• Assess patient understanding of their condition and readiness to change. Provide patient with education about the disease process; offer tools and skills needed to achieve self-management goals.
• Respond to emergent health issues; assess, advise and provide first aid and Basic Life Support as needed. Maintain joint accountability to the clinic team and to the Care and Connect program as a whole, to include supporting colleagues throughout the organization either on site or remotely as needed.
• Work with team members and leaders to define and reassess priorities and plan, communicate, and execute care delivery.
• Participate in Quality and Performance Improvement efforts at the site/program to improve patient outcomes as requested.
• May serve as a preceptor for new staff and/or students who are assigned to Medical Teams as part of their professional education. Support safeguarding of program participants and employees through promoting an environment of awareness and upholding the code of conduct provisions of Medical Teams Protection from Sexual Exploitation & Abuse, Child & Vulnerable Adult protection, and harassment-free workplace policies.
• Collaborate with patients, clients, staff, and community partners to identify and address the ways that systemic racism and other forms of compounding bias are root causes of disparities in clinical/mental health and homelessness in Black, Indigenous, and other communities of color and other communities that have experienced historical and structural oppression.
• Support safeguarding of program participants and employees through promoting an environment of awareness and upholding the code of conduct provisions of Medical Teams Protection from Sexual Exploitation & Abuse, Child & Vulnerable Adult protection, and harassment-free workplace policies. Incumbents will remain alert and responsive to any child and adult safeguarding risks, acquire relevant knowledge and skills which will enable you to promote strong safeguarding practices, understand the child and adult safeguarding policy and procedures, and conduct yourself in a manner consistent with the Safeguarding code of conduct and behavior expectations. 

Other Duties:

• In collaboration with the Clinic Manager, Clinic Assistant and clinical volunteers, monitor, track and follow-up with patients to ensure care needs are met and referrals are completed and followed through using practice unit Resource Guides and electronic care coordination platforms.
• Utilize volunteers and delegate tasks when appropriate.
• Additional duties as assigned.

Qualifications

EDUCATION, LICENSES, & CERTIFICATIONS

Required:

• Graduation from an accredited nursing program.
• State RN license in good standing.
• Active CPR/BLS Certificate.

Preferred:

• Bachelor’s degree in Nursing.
• Master’s degree in Public Health.
• Care Coordination or Case Management Certification a plus.
• Bilingual, Spanish preferred.

EXPERIENCE

• Minimum 2 years’ work experience working in a fast-paced healthcare setting preferred.
• Experience working on interdisciplinary teams.

KNOWLEDGE, SKILLS & ABILITIES

Knowledge:

• Knowledge and understanding of a broad range of physical and mental health conditions throughout the lifespan.
• Knowledge of best practices in primary prevention, screening, and patient education.
• Knowledge of medical terminology and clinical procedures; anatomy, physiology, biology, human growth and development; asepsis and universal precautions; medical documentation.
• Familiar with the principles of trauma-informed practice and culturally responsive care.

Skills:

• Skills in written and verbal communication in English and basic math. Ability to comply with HIPAA/Confidentiality policies and procedures and to handle confidential and sensitive patient and staff information.
• Moderately skilled with office software: Microsoft Word Suite, Excel, and PowerPoint and electronic health records systems.
• Moderately skilled with communication software: Outlook, MS Teams, Internet, CRM. Prior experience with Open Dental is a plus.
• Self-starter, quick and curious learner, detail oriented, highly organized, and able to work independently and in remote settings.

• Ability to work effectively with patients and communicate effectively and respectfully with people of varied cultures, ethnic, language (including use of interpreters), educational and socio-economic backgrounds, as well as individuals with disabilities and contagious diseases.

• Ability to work a flexible schedule and weekends as needed.
• Ability to travel to various rural and urban locations within Oregon or Washington.

Travel Requirements:

• This position is up to 24 hours per week, an hourly position which includes frequent regional travel of up to 2-hours, with occasional overnight travel as needed.
