Job Opportunity:

We currently have open needs for LVN Home Health professionals on a per-diem basis.

Expectations and Responsibilities:

In this role, you will be responsible for providing home health visits to various patients in their homes. Your duties will include conducting follow-up visits with patients with home health needs. We also offer training and benefits to our team members.

The Role:

The Licensed Vocational Nurse/LVN is an essential member of our healthcare team. As an LVN, you will provide skilled nursing care and prescribed medical treatment to patients in compliance with the Nurse Practice Act in the State of California. You will work under the supervision of a Nurse Practitioner or Physician.

Other Responsibilities:

• Providing nursing care within the scope of LVN practice
• Gathering relevant data for patients' individual needs
• Assisting with patient and family education alongside the RN
• Performing basic medical care tasks, such as changing bandages, administering medication, and collecting specimens
• Ensuring the health, comfort, and safety of patients by assisting with bathing, feeding, and dressing
• Monitoring and reporting any changes in patient's condition to the supervisor

Qualifications:

• Must have at least one year of active clinical experience
• Active LVN license in the state of California
• Active driver's license with a clean driving record
